摘要
萆薢抗痛风的作用机制主要包括降尿酸、抑制炎症、免疫调节、肾脏保护等。目前,关于萆薢抗痛风的相关研究存在的不足主要包括:(1)萆薢治疗痛风的研究多为复方中药,也有部分萆薢提取物,单一成分研究较少,而复杂成分的有效性和安全性需要进一步研究证实;(2)相关研究多偏向于萆薢治疗痛风性关节炎,针对痛风性肾病的报道较少,且偏向实验研究,临床研究较少。今后,可加强萆薢分离单体成分在痛风中的应用及其可能机制的研究,从而开发安全有效治疗痛风的药物;可开展相关前瞻性随机对照研究,以证实萆薢对痛风性肾病患者的肾保护作用,是否能改善患者远期预后等。另外,萆薢药理作用还未完全清楚,除了抗感染、免疫改善、抑制尿酸等作用外,是否还经由其他途径作用于痛风有待日后深入分析。
The mechanism of anti-gout of Bixie(Rhizoma Dioscoreae Hypoglaucae) mainly includes uric acid reduction,inflammation inhibition,immune regulation,kidney protection,etc.At present,the shortcomings of the research on the anti-gout of Bixie mainly are as follows:(1) The research on the treatment of gout with Bixie is mostly compound Chinese medicine,and there are some Bixie extracts,and there are few studies on single component,and the effectiveness and safety of complex components need to be further studied;(2) The related research is more biased to the treatment of gouty arthritis with Bixie,and there are few reports on gouty nephropathy,and they are biased to experimental research,and there are few clinical studies.In the future,the application of Bixie isolated monomers to gout and its possible mechanisms should be strengthened in order to develop safe and effective drugs for gout treatment.Prospective randomized controlled studies should be conducted to confirm the renal protection of Bixie in patients with gouty nephropathy and whether it improves the long-term prognosis of patients.In addition,the pharmacological effects of Bixie are not fully understood.Whether it also acts on gout through other pathways,in addition to its anti-infective,immune-improving,uric acid-suppressing,and other effects,remains to be further analyzed.
