清脉解毒法治疗糖尿病足急性期的临床疗效及对相关炎症因子的影响

Clinical efficacy of pulse clearing and detoxification therapeutic method in the treatment of acute stage of diabetic foot and its influence on related inflammatory factors

目的探讨清脉解毒法治疗糖尿病足急性期的临床疗效及对相关炎症因子的影响。方法选取2021年2月至2023年4月蚌埠市中医医院收治的60例糖尿病足急性期住院患者,根据是否应用清脉解毒法将患者分为对照组(采取常规基础治疗)和治疗组(在常规基础治疗的基础上采取清脉解毒法进行治疗),每组30例。比较两组患者治疗4周后的临床疗效、创面愈合率及截肢率;治疗前与治疗4周后的创面证候积分、疼痛程度[视觉模拟评分法(VAS)评分]、炎症因子[C反应蛋白(CRP)、肿瘤坏死因子α(TNF-α)、白细胞介素(IL)-6]水平及不良反应发生情况。结果治疗4周后,治疗组患者的治疗总有效率、创面愈合率均高于对照组患者,截肢率、VAS评分、CRP水平、IL-6水平及TNF-α水平均低于对照组患者(P﹤0.05);两组患者的创面证候积分比较,差异均有统计学意义(P﹤0.05)。两组患者在治疗期间均未发生严重的肝肾功能异常、过敏反应及胃肠道反应等。结论在糖尿病足急性期的治疗中,清脉解毒法可改善患者的患足疼痛感,促进创面愈合,降低截肢率,抑制患足炎症,提高临床疗效,值得临床推广使用。

Objective To explore the clinical efficacy of the pulse clearing and detoxification therapeutic method in the treatment of acute stage of diabetic foot and its influence on related inflammatory factors.Method A total of 60 hospitalized patients with acute stage of diabetic foot admitted to Bengbu Traditional Chinese Medicine Hospital from February 2021 to April 2023 were selected.The patients were divided into two groups based on whether they were treated with pulse clearing and detoxification therapeutic method,30 patients in each group.The control group was given routine basic treatment,while the treatment group was given the pulse clearing and detoxification therapeutic method on the basis of routine basic treatment.The clinical efficacy,wound healing rate,and amputation rate after 4 weeks of treatment were compared between the two groups.The wound syndrome scores,pain severity[visual analogue scale(VAS)score],the serum levels of inflammatory factors[C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin(IL)-6]and adverse reaction occurrence before and after 4 weeks of treatment were compared between the two groups.Result After 4 weeks of treatment,the total effective rate and wound healing rate of the treatment group were higher than those of the control group,the amputation rate,VAS scores,CRP level,IL-6 level,and TNF-αlevel of the treatment group were all lower than those of the control group(P<0.05);the difference in wound syndrome scores between the two groups of patients was statistically significant(P<0.05).Both groups of patients did not experience severe dysfunction of liver and kidney,allergic reactions or gastrointestinal reactions during the treatment period.Conclusion In the treatment of acute stage of diabetes foot,the method of clearing pulse and detoxification can improve the patient's foot pain,promote wound healing,reduce amputation rate,inhibit inflammation of foot,and improve clinical efficacy,which is worthy of clinical promotion and use.
