颈动脉溃疡斑块与缺血性脑卒中发生的关系

Relationship between carotid artery ulcer plaques and ischemic stroke

目的探讨颈动脉溃疡斑块与缺血性脑卒中(IS)发生的关系。方法收集2018年6月至2020年12月于北京市仁和医院就诊的38例缺血性脑血管疾病患者的临床资料,所有患者均接受颈动脉彩色多普勒超声检查(CDU)及头颅磁共振成像(MRI)检查,分析患者是否存在颈动脉溃疡斑及其与IS发生的关系。结果38例缺血性脑血管病患者共发现颈动脉溃疡斑块40个,其中,低回声斑块27个(67.50%),低回声为主的混合回声斑块13个(32.50%)。29例患者确诊为IS,阳性率为76.31%(29/38),其中,低回声斑块21个(72.41%),低回声为主的混合回声斑块8个(27.59%)。结论颈动脉溃疡斑块与IS发生关系密切,低回声溃疡斑块发生率高于低回声为主的混合回声斑块,低回声溃疡斑IS发生率高于低回声为主的混合回声斑块。

Objective To investigate the relationship the between carotid artery ulcer plaques and ischemic stroke(IS).Method Clinical data of 38 patients with ischemic cerebrovascular disease who were treated in Beijing Renhe Hospital from June 2018 to December 2020 were collected.All patients underwent carotid color Doppler ultrasonography(CDU)and head magnetic resonance imaging(MRI),whether the patients had carotid artery ulcer plaques and their relationship with the occurrence of IS were analyzed.Result A total of 40 carotid artery ulcer plaques were found in 38 patients with ischemic cerebrovascular disease,including 27 hypoechoic plaques(67.50%)and 13 mixed echoic plaques(32.50%)with predominantly hypoechoic features.29 patients were diagnosed with IS,the positive rate was 76.31%(29/38),including 21 hypoechoic plaques(72.41%)and 8 mixed echoic plaques(27.59%)with predominantly hypoechoic features.Conclusion The relationship between carotid artery ulcer plaques and IS is closely related,with higher incidence of low-echo ulcer plaques than mixed echo plaques dominated by low echo.The incidence of IS in low echo ulcer plaques is higher than that in mixed echo plaques dominated by low echo plaques.
