两种电针方案结合红外线照射治疗术后尿潴留临床研究

Clinical Study on Two Electroacupuncture Schemes Combined with Infrared Radiation for Postoperative Urinary Retention

摘要 目的:观察两种电针方案结合红外线照射治疗术后尿潴留的临床效果。方法:将90例术后尿潴留患者按随机数字表法分为对照组、治疗组A、治疗组B,每组30例。对照组行红外线照射治疗,治疗组A行电针(高频)+红外线照射治疗,治疗组B行电针(低频)+红外线照射治疗。评价3组临床疗效,比较3组排尿情况和尿动力学指标。结果:治疗组A、治疗组B总有效率均高于对照组(P<0.05),治疗组B总有效率高于治疗组A(P<0.05)。治疗后,治疗组A、治疗组B首次排尿时间、排尿困难评分均低于对照组,首次排尿量、治疗后2 h排尿量均高于对照组(P<0.05)。且治疗组B首次排尿时间、排尿困难评分低于治疗组A,首次排尿量、治疗后2 h排尿量均高于治疗组A(P<0.05)。治疗后,3组膀胱容量、平均尿流速、逼尿肌压力、最大尿流速均较治疗前升高(P<0.05),治疗组A、治疗组B膀胱容量、平均尿流速、逼尿肌压力、最大尿流速均高于对照组(P<0.05),治疗组B各项指标均高于治疗组A(P<0.05)。结论:电针结合红外线照射治疗术后尿潴留,能增强治疗效果,促进排尿及尿动力学指标改善,且相较于高频电针方案,低频电针方案疗效更优。 Objective:To observe the clinical effect of two electroacupuncture schemes combined with infrared radiation for postoperative urinary retention.Methods:A total of 90 patients with postoperative urinary retention were divided into the control group,the treatment group A and the treatment group B,with 30 cases in each group.The control group received infrared radiation therapy,the treatment group A received electroacupuncture(high-frequency) and infrared radiation therapy,and the treatment group B received electroacupuncture(low-frequency) and infrared radiation therapy.The clinical effects of the three groups were evaluated,and the urination status and urodynamic indicators were compared in the three groups.Results:The total effective rates in the treatment group A and the treatment group B were higher than that in the control group(P<0.05),and the total effective rate in the treatment group B was higher than that in the treatment group A(P<0.05).After treatment,the scores of the first urination time and urination difficulty in the treatment group A and the treatment group B were lower than those in the control group,and the first urination volume and 2-hour urination volume were higher than those in the control group(P<0.05).The scores of the first urination time and urination difficulty in the treatment group B were lower than those in the treatment group A,and the first urination volume and 2-hour urination volume after treatment were higher than those in the treatment group A(P<0.05).After treatment,the bladder volume,average urine flow rates,detrusor pressure,and maximum urine flow rates in the three groups were all increased when compared with those before treatment(P<0.05);the bladder volume,average urine flow rates,detrusor pressure,and maximum urine flow rates in the treatment group A and the treatment group B were higher than those in the control group(P<0.05),and all indicators in the treatment group B were higher than those in the treatment group A(P<0.05).Conclusion:Electroacupuncture combined with infrared radiation treatment for postoperative urinary retention can enhance the curative effect,promote the improvement of urination and urodynamic indicators,and low-frequency electroacupuncture is more effective than high-frequency electroacupuncture.
