河南省发热门诊护理质量敏感指标的构建

Construction of Sensitive Indicators in Fever Clinics in Henan Province

目的建立一套科学、敏感、具有可操作性和代表性的发热门诊护理质量敏感指标,为护理管理者进行发热门诊护理质量评价与改进提供参考。方法以“结构-过程-结果”三维质量模型为理论基础,通过文献回顾分析初步筛选发热门诊护理质量敏感指标,2022年10—12月,运用德尔菲专家函询法构建发热门诊护理质量敏感指标体系。结果构建的发热门诊护理质量敏感指标包含3个一级指标,传染性专科护士占比、护理人员专科培训合格率、空气消毒设施设备完好率、环境卫生学监测合格率等19个二级指标。2轮咨询专家积极系数分别为90.91%和100.00%,权威系数为0.78、0.79,肯德尔和谐系数为0.203~0.715(P<0.01)。变异系数为0.08~0.22。结论该研究构建的发热门诊护理质量敏感指标具有发热门诊护理特色,实用性强,能够科学、全面敏感衡量发热门诊护理工作质量,能够更好地指导发热门诊护理质量控制工作。

Objective Establish a set of scientific,sensitive,operable and representative nursing quality sensitive indicators in fever clinics to provide reference for nursing managers to evaluate and improve the nursing quality in fever clinics.Methods Based on the three-dimensional quality model of“structure-process-result”as the theory,the nursing quality sensitive indicators of fever clinics were preliminarily screened through literature review,analysis and group discussion.From October to December 2022 the fever clinic nursing quality sensitive index system was constructed by using the Delphi expert letter consultation method.Results The sensitive index of nursing quality in fever clinics includes 3 first-level indicators,and 19 second-level indicators,including the proportion of infectious specialty nurses,qualified rate of nursing staff training,intact rate of air disinfection facilities and equipment,and qualified rate of environmental hygiene monitoring.Positive factors of two consultation experts were 90.91%and 100.00%,authority coefficient was 0.78,0.79,and Kendall harmony coefficient was 0.203-0.715(P<0.01).The coefficient of variation ranged from 0.08-0.22.Conclusion The sensitive index of nursing quality of fever clinic constructed in this study has the characteristics of fever clinic nursing,strong practicability,can scientifically and can measure the quality of nursing work of fever clinic comprehensively,and can better guide the quality control work of fever clinic.
