血清CTRP9、TGF-β2、ANGPTL3水平与糖尿病视网膜病变的相关性分析及临床意义

Correlation Analysis and Clinical Significance of Serum CTRP9,TGF-β2,ANGPTL3 Levels with Diabetic Retinopathy

摘要 目的探讨血清C1q肿瘤坏死因子相关蛋白9(CTRP9)、转化生长因子-β2(TGF-β2)、血管生成素样蛋白3(ANGPTL3)水平与糖尿病视网膜病变(DR)的相关性及临床意义。方法回顾性选取2021年9月至2023年9月医院收治的100例DR患者,其中增殖性视网膜病变(PDR)39例,非增殖性视网膜病变(NPDR)61例,分别纳入PDR组、NPDR组,另选取同期糖尿病不伴有视网膜病变患者50例,纳入对照组。比较3组血清CTRP9、TGF-β2、ANGPTL3水平,分析血清各指标与DR病情严重程度的相关性及联合检测诊断价值,并分析各指标不同水平患者发生DR的危险度。结果3组血清CTRP9、TGF-β2、ANGPTL3水平比较:PDR组>NPDR组>对照组,且各指标水平与DR病情严重程度均呈正相关(P<0.05)。入院时血清CTRP9、TGF-β2、ANGPTL3水平联合诊断DR的曲线下面积为0.899,最佳诊断敏感度为91.00%,最佳诊断特异度为88.00%,约登指数为0.790,且各指标高水平患者发生DR的危险度是低水平的1.419倍、1.239倍、1.381倍(P<0.05)。结论CTRP9、TGF-β2、ANGPTL3在DR患者血清中均呈异常高表达,各指标水平与病情严重程度均呈正相关,且联合检测对DR具有一定诊断价值,可作为临床诊断疾病、评估病情的辅助指标。 Objective To investigate the correlation and clinical significance of serum C1q tumor necrosis factor-related protein 9(CTRP9),transforming growth factor-β2(TGF-β2),angiopoietin-like protein 3(ANGPTL3)levels with diabetic retinopathy(DR).Methods A total of 100 patients with DR Admitted to the hospital from September 2021 to September 2023 were retrospectively selected,including 39 cases of proliferative diabetic retinopathy(PDR)and 61 cases of non-proliferative diabetic retinopathy(NPDR),which were included in the PDR group and NPDR group,and 50 cases of diabetic patients without retinopathy during the same period were included in the control group.The serum levels of CTRP9,TGF-β2 and ANGPTL3 in the 3 groups were compared,and the correlation between serum indicators and the severity of DR disease and the diagnostic value of combined detection were analyzed,and the risk of DR in patients with different levels of each indicator was analyzed.Results The levels of CTRP9,TGF-β2 and ANGPTL3 in 3 groups were compared,the PDR group>the NPDR group>the control group,and the levels of each index were positively correlated with the severity of DR Disease(P<0.05).At admission,the area under the curve of serum CTRP9,TGF-β2 and ANGPTL3 in the combined diagnosis of DR was 0.899,the best diagnostic sensitivity was 91.00%,the best diagnostic specificity was 88.00%,and the Yodon index was 0.790.The risk of DR in patients with high levels of each index was 1.419 times,1.239 times,1.381 times higher than that in patients with low levels(P<0.05).Conclusion CTRP9,TGF-β2 and ANGPTL3 are all abnormally high expressed in the serum of DR patients,and the levels of each indicator are positively correlated with the severity of the disease,and the combined detection has certain diagnostic value for DR,and can be used as an auxiliary indicator for clinical diagnosis and evaluation of the disease.
