雷公藤制剂及其提取物在肾移植领域应用的研究进展

Research Progress of Leigongteng(Tripterygium wilfordii)Preparation and Its Extract in Kidney Transplantation

肾移植术后管理取得了阶段性发展,然而移植患者/肾的长期存活率尚未有效延长,与肾移植术后诸多并发症密切相关。肾移植后急慢性肾损伤,包括缺血再灌注损伤(Ischemia reperfusion injury, IRI)、排斥反应、蛋白尿和免疫抑制毒性等均显著影响患者的远期生存及预后。传统中药材雷公藤具有祛风除湿、通络止痛及活血消肿的功效,现代药理研究证实其具有抗炎、免疫抑制及抗肿瘤等活性。现今诸多学者将雷公藤制剂及其提取物用于肾移植术后的管理方案中,虽初见成效,但在实际使用中仍存在亟待解决的问题。以雷公藤制剂及其提取物在肾移植领域的用药方案为基础,总结其发挥作用、机制及其不良反应,进而明确雷公藤制剂及其提取物在肾移植领域应用的安全性、有效性及应用前景,为提升移植患者/肾的长期存活率提供借鉴参考。

The postoperative management of kidney transplantation has achieved phased development,but the long-term survival rate of transplant patients/kidneys has not been effectively extended,which is closely related to many complications after kidney transplantation.Acute and chronic renal injury after renal transplantation,including ischemia reperfusion injury,rejection,albuminuria and immunosuppressive toxicity,significantly affects long-term survival and prognosis of patients.The traditional Chinese herbal medicine Leigongteng(Tripterygium wilfordii)has the effects of dispelling wind and dampness,relieving pain,promoting blood circulation and detumorizing.Modern pharmacological studies have confirmed that it has anti-inflammatory,immunosuppressive and anti-tumor activities.At present,many scholars have applied Leigongteng(Tripterygium wilfordii)preparation and its extract in the management program after kidney transplantation.Although the initial results have been achieved,there are still some problems to be solved in the practical application.Based on the medication regimen of Leigongteng(Tripterygium wilfordii)preparation and its extracts in the field of kidney transplantation,this paper summarized their functions,mechanisms and adverse reactions,and further clarified the safety,effectiveness and application prospect of Leigongteng(Tripterygium wilfordii)preparation and its extracts in the field of kidney transplantation,providing reference for improving the long-term survival rate of transplant patients/kidneys.
