苏合香丸对老年脑卒中患者肺部感染肺功能和免疫功能的影响

Effect of Suhexiang Pills(苏合香丸)on Pulmonary Function and Immune Function in Elderly Cerebralvascular Accident Patients with Pulmonary Infection

目的 探讨苏合香丸辅助治疗对老年脑卒中患者肺部感染肺功能和免疫功能的影响。方法 选取医院重症加强病房(Intensive care unit, ICU)2019年1月—2020年1月收治的重症脑卒中(Cerebralvascular accident, CVA)患者120例,根据治疗方法的不同分为观察组和对照组。对照组给予注射用盐酸头孢吡肟和胃管给复方甘草口服溶液,观察组在对照组基础上加用苏合香丸,均持续治疗14 d。统计并比较两组疗效,采用流式细胞仪测定外周血T淋巴细胞亚群,包括CD_(4)^(+)、CD_(8)^(+)T淋巴细胞和CD_(4)^(+)CD_(8)^(+)T淋巴细胞比值,经肺功能仪检测患者的肺功能指标[FEV1/FVC、 FEV1%pred、最大自主通气量(MVV)、血肌酐],记录并比较两组不良反应发生情况。结果 观察组治疗总有效率为100.00%(60/60),显著高于对照组的83.33%(50/60)(P<0.05)。观察组不良反应发生率为6.67%(4/60),显著低于对照组的11.67%(7/60)(P<0.05)。治疗后,观察组CD_(4)^(+)和CD_(4)^(+)CD_(8)^(+)水平改善效果显著优于对照组(P<0.05)。疗后,观察组FEV1/FVC、FEV1%pred和MVV高于对照组,血肌酐较低于对照组(P<0.05)。结论 苏合香丸辅助治疗对老年脑卒中患者肺部感染的疗效显著,快速缓解患者体征与临床症状,不良反应较少,可以显著改善患者机体的免疫功能、肺功能。

Objective To investigate the effect of Suhexiang Pills(苏合香丸)adjuvant therapy on lung function and immune function of elderly cerebralvascular accident(CVA)patients with pulmonary infection.Methods A total of 120 severe CVA patients admitted to the ICU of the hospital from January 2019 to January 2020 were selected and divided into observation group and control group according to different treatment methods.The control group was given cefepime hydrochloride for injection and Compound Licorice Oral Solution by gastric tube,and the observation group was given Suhexiang Pills for 14 days.The curative effects of the two groups were counted and compared,and the peripheral blood T lymphocyte subsets,including CD_(4)^(+),CD_(8)^(+) T lymphocytes and CD_(4)^(+) CD_(8)^(+) T lymphocyte ratios,were measured by flow cytometry.:Forced expiratory volume in the first second(FEV1)/forced vital capacity(FVC),FEV1%pred,maximum spontaneous ventilation(MVV),serum creatinine and the occurrence of adverse reactions between the two groups were recorded and compared.Results The total effective rate of the observation group was 100.00%,which was significantly higher than that of the control group(66.67%)(P<0.05).The incidence of adverse reactions in the observation group was 6.67%(4/60),which was significantly lower than 11.67%(7/60)in the control group(P<0.05).After treatment,the improvement effect of CD_(4)^(+) and CD_(4)^(+) CD_(8)^(+) in the observation group was significantly better than that in the control group(P<0.05).After treatment,the values of FEV1/FVC,FEV1%pred and MVV of the observation group were higher than those of the control group,and the serum creatinine was lower than that of the control group(P<0.05).Conclusion Suhexiang Pills adjuvant therapy has significant curative effect on pulmonary infection in elderly CVA patients,quickly relieves the patient's physical signs and clinical symptoms,and has fewer adverse reactions,and can significantly improve the immune function and pulmonary function of the patient.
