温阳消瘿方治疗桥本甲状腺炎临床疗效及对患者TPOAb、TGAb抗体滴度的影响

Clinical Effect of Wenyang Xiaoying Formula(温阳消瘿方)on Hashimoto's Thyroiditis and Its Influence on TPOAb and TGAb Titers

目的 观察温阳消瘿方治疗桥本甲状腺炎临床效果及对患者TPOAb、TGAb抗体滴度的影响。方法 将医院内分泌科收治的89例桥本甲状腺炎患者(2021年6月—2022年6月)纳入本次试验,以随机数字法将纳入患者分成对照组(44例)与试验组(45例)两组,对照组患者均接受西医常规治疗,试验组患者在对照组治疗基础上结合温阳消瘿方治疗,比较两组患者的临床疗效、治疗前后血清抗甲状腺过氧化物酶抗体(TPOAb)和抗甲状腺球蛋白抗体(TGAb)滴度、中医证候积分(颈前肿大、神疲乏力、畏寒肢冷、腰膝酸软)、甲状腺左右叶前后径及峡部厚度以及治疗不良反应。结果 试验组较对照组的治疗总有效率更高(P<0.05)。两组患者治疗前TPOAb、TGAb抗体滴度、中医证候积分(颈前肿大、神疲乏力、畏寒肢冷、腰膝酸软)、甲状腺左右叶前后径及峡部厚度比较(P>0.05),具有可比性;治疗后两组患者TPOAb、TGAb抗体滴度、中医证候积分均明显降低,试验组较对照组TPOAb、TGAb抗体滴度、中医证候积分降低幅度更大(P<0.05);治疗后甲状腺左右叶前后径及峡部厚度变化差异无统计学意义(P>0.05)。结论 温阳消瘿方治疗桥本甲状腺炎临床效果显著,患者症状显著好转,甲状腺抗体滴度明显改善,治疗过程中均未见不良反应,安全有效。

Objective To observe the clinical effect of Wenyang Xiaoying Formula(温阳消瘿方) on Hashimoto's thyroiditis and its influence on anti thyroid peroxidase antibody(TPOAb) and anti thyroglobulin antibody(TGAb) titers.Methods Eighty-nine patients with Hashimoto's thyroiditis admitted to the endocrine department of the hospital(from June 2021 to June 2022) were included in this experiment.The patients were randomly divided into two groups:the control group(44 cases) and the test group(45 cases).The patients in the control group were treated with conventional western medicine.The patients in the test group were treated with Wenyang Xiaoying Formula on the basis of the treatment in the control group.The data of the two groups was compared before and after treatment.The titers of TPOAb and TGAb,TCM syndrome scores(anterior cervical swelling,fatigue,cold and cold limbs,waist and knee stiffness) and adverse reactions of treatment were compared.Results The total effective rate of the test group was higher than that of the control group(P<0.05).Before treatment,the titers of TPOAb and TGAb,TCM syndrome scores(anterior cervical swelling,fatigue,cold chills,waist and knee stiffness) and other indicators of the two groups were not significant(P>0.05).After treatment,the titers of TPOAb and TGAb and TCM syndrome scores of the two groups were significantly reduced,and the titers of TPOAb and TGAb and TCM syndrome scores of the test group were significantly reduced compared with those of the control group.The difference was statistically significant(P<0.05).There is no statistically significant difference in the changes of the anterior-posterior diameter and isthmus thickness between the left and right lobes of the thyroid after treatment(P>0.05).Conclusion Wenyang Xiaoying Formula has a significant clinical effect on Hashimoto's thyroiditis.The symptoms of the patients are improved significantly,and the thyroid antibody titer has been improved significantly.No adverse reactions were observed during the treatment.It is safe and effective.
