中西医结合治疗慢性肛周湿疹临床观察

Integrated Traditional Chinese and Western Medicine in the Treatment of Chronic Perianal Eczema

目的 观察自制肛肠科熏洗方结合复方克霉唑乳膏(Ⅱ)外涂、普济痔疮栓纳肛治疗慢性肛周湿疹(PE)的临床效果。方法 采用随机数字表法将60例慢性PE患者分对照组和观察组,各30例。对照组予复方克霉唑乳膏(Ⅱ)外涂,观察组予自制肛肠科熏洗方熏洗坐浴,联合复方克霉唑乳膏(Ⅱ)外涂、普济痔疮栓纳肛。两组皆以7 d为1个疗程,经2个疗程,观察两组患者肛周皮损面积、瘙痒程度、皮损形态、渗液等各方面评分,对比两组复发率与临床治疗效果。结果 两组肛周皮损面积、瘙痒程度、皮损形态与渗液评分方面皆大幅改善,且观察组低于对照组(P<0.05);观察组总有效率90.0%(27/30)高于对照组的66.7%(20/30)(P<0.05);观察组复发率6.7%(2/30)低于对照组的36.7%(11/30)(P<0.05)。结论 中西医结合疗法用于慢性PE治疗,可获得确切效果,且不易复发,在临床上有推行价值。

Objective To observe the clinical effect of self-made anorectal fumigation and washing prescription combined with compound clotrimazole cream Ⅱ and hemorrhoids suppository in the treatment of chronic perianal eczema(PE).Methods Sixty patients with chronic PE were randomly selected.They were divided into control group and observation group.The control group was coated with compound clotrimazole cream Ⅱ,and the observation group was treated with self-made anorectal fumigation and bath formula.Combined with compound clotrimazole cream Ⅱ external coating and Puji hemorrhoid suppository anus.Both groups had a 7-day treatment.After 2 coursers of treatment,the scores of perianal skin lesion area,pruritus degree,skin lesion morphology and exudation were observed in the 2 groups.The recurrence rate and clinical treatment effect were compared between the two groups.Results Perianal skin area,pruritus degree,skin shape and permeability score were significantly improved in both groups,and the observation group was lower than the control group(P<0.05).The total effective rate of observation group was 90.0%(27/30),which was higher than that of control group(66.7%,20/30)(P<0.05).The recurrence rate of the observation group was 6.7%(2/30),which was lower than 36.7%(11/30) in the control group(P<0.05).Conclusion The combination of traditional Chinese and Western medicine in the treatment of chronic PE can obtain the exact effect,is not easy to relapse,and has clinical value.
