IVIM和增强T1mapping成像对肺癌化疗疗效的预测价值

摘要 目的探讨基于体素内不相干运动扩散加权成像(IVIM-DWI)及增强T_(1)mapping序列的定量参数在预测肺癌化疗疗效中的作用。方法选取52例经病理确诊并至少完成两个周期化疗的晚期肺癌患者资料,根据治疗效果分为化疗敏感组与不敏感组。化疗前均行IVIM-DWI及增强T_(1)mapping序列检查,测量、计算化疗敏感组与不敏感组病变的D值、D^(*)值、f值、增强T_(1)mapping的各参数值(T_(1)pre、T_(1)post、ΔT_(1)%)及细胞外体积(ECV)分数,并进行统计学分析,采用受试者工作特征曲线(ROC曲线)评估各参数预测肺癌化疗疗效的效能。结果敏感组治疗前T_(1)post明显低于不敏感组(t=-4.96,P<0.001),ΔT_(1)%及ECV分数明显高于不敏感组(t=5.40,P<0.001;t=5.64,P<0.001)。ROC曲线结果显示ECV分数在肺癌化疗疗效预测中的AUC最高(AUC=0.880)。ECV分数与T_(1)post或ΔT_(1)%之间的AUC没有明显差异。D、D^(*)、f、T_(1)pre两组间比较,差异无统计学意义(P>0.05)。结论敏感组通常比不敏感组有更低的T_(1)post值、更高的ΔT_(1)%值和ECV分数,T_(1)post、ΔT_(1)%和ECV分数对化疗前预测晚期肺癌疗效具有一定作用。 Objective To investigate the role of quantitative parameters obtained based on intravoxel incoherent motion-diffusion weighted imaging(IVIM-DWI)and enhanced T_(1)mapping sequences in predicting the chemotherapy efficacy in lung cancer.Methods Fifty-two patients with advanced lung cancer who were diagnosed by pathology and had completed at least two cycles of chemotherapy in our hospital were selected and divided into a treatment-sensitive group and an insensitive group according to their treatment effects.Patients underwent IVIM-DWI and enhanced T_(1)mapping sequences prior to treatment,and D,D^(*),fvalues,values of enhanced T_(1)mapping parameters(T_(1)pre,T_(1)post,ΔT_(1)%),and extracellular volume(ECV)fraction were measured and calculated.Statistical analysis was performed.Results The T_(1)post values of pre-treatment lesions in the sensitive group were significantly lower than those in the insensitive group(t=-4.96,P<0.001),and theΔT_(1)%and ECV fractions of pre-treatment lesions were significantly higher than those in the insensitive group(t=5.40,P<0.001;t=5.64,P<0.001).The ROC curves showed that ECV fractions had the highest AUC in predicting chemotherapy efficacy in lung cancer(AUC=0.880),and there was no significant difference in AUC between ECV fractions and T_(1)post orΔT_(1)%.D,D^(*),f,and T_(1)pre showed no statistically significant differences between the two groups(P>0.05).Conclusion T_(1)post,ΔT_(1)%and ECV fraction have a role in predicting the efficacy of advanced lung cancer before chemotherapy,and the sensitive group usually has lower T_(1)post values,higherΔT_(1)%values and ECV fraction than the insensitive group.
