柴胡疏肝散合左金丸治疗肝胃郁热型难治性胃食管反流病临床研究

Chaihu Shugan Powder and Zuojin Pill in Treating Refractory Gastroesophageal Reflux Disease of Liver and Stomach Stagnation Heat Type

目的观察柴胡疏肝散合左金丸治疗难治性胃食管反流病(RGERD)(肝胃郁热型)的临床疗效。方法将112例符合标准的患者随机分为治疗组及对照组各56例,对照组予雷贝拉唑及莫沙必利,治疗组在对照组基础上加服柴胡疏肝散合左金丸。治疗8周后,观察2组患者的症状、内镜下食管炎症、Gerd⁃Q积分、SAS评分、SDS评分、GAS的变化及治疗期间不良反应发生情况。结果治疗组总有效率优于对照组,差异有统计学意义(P<0.05)。治疗后,2组中医证候、内镜下食管黏膜炎症、Gerd⁃Q、SAS评分、SDS评分均比治疗前降低(P<0.05),2组GAS水平均上升(P<0.05),治疗组更显著(P<0.05)。2组患者均未见明显不良反应。结论柴胡疏肝散合左金丸治疗肝胃郁热型RGERD疗效显著,安全性良好。

Objective To observe the clinical efficacy of Chaihu Shugan powder combined with Zuojin pills in the treatment of refractory gastroesophageal reflux disease(RGERD)(liver and stomach stagnation heat type).Methods A total of 112 patients met the criteria were randomly divided into treatment group and control group,with 56 cases in each group.The control group was given rabeprazole and mosapride,and the treatment group was given Chaihu Shugan powder combined with Zuojin pills on the basis of control group.After 8 weeks of treatment,the symptoms,endoscopic esophageal inflammation,Gerd⁃Q scores,SAS scores,SDS scores,GAS changes and the occurrence of adverse reactions during treatment in 2 groups were observed.Results The total effective rate of the treatment group was better than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the scores of TCM syndrome,esophageal mucosal inflammation under endoscopy,Gerd⁃Q,SAS and SDS in 2 groups were decreased compared with those before treatment(P<0.05),and GAS levels in 2 groups were increased(P<0.05),and it was more significantly in the treatment group(P<0.05).No obvious adverse reactions were observed in both groups.Conclusion Chaihu Shugan powder combined with Zuojin pills is effective and safe in the treatment of RGERD of liver and stomach stagnation heat type.
