基于fMRI的角刺法联合经筋排刺法治疗难治性周围性面瘫临床观察

Angle Prick Method Combined with Meridian Sinew Row Acupuncture in the Treatment of Refractory Peripheral Facial Paralysis

目的探究基于功能性磁共振成像(fMRI)的角刺法联合经筋排刺法治疗难治性周围性面瘫的临床效果。方法随机将76例难治性周围性面瘫患者分为对照组和观察组,每组38例,分别给予常规针刺、基于fMRI的角刺法联合经筋排刺法治疗,持续4周,比较2组面神经功能(Sunnybrook)评分、面神经麻痹症状评分以及面部肌电图指标,计算愈显率。结果观察组治疗后Sunnybrook评分高于对照组(P<0.05),面神经麻痹症状评分低于对照组(P<0.05),面部肌电图瞬目反射潜伏期和面神经传导潜伏期、波幅参数优于对照组(P<0.05),愈显率高于对照组(P<0.05)。结论基于fMRI的角刺法联合经筋排刺法能够有效恢复难治性周围性面瘫患者面神经功能,改善临床症状,临床疗效优越。

Objective To explore the clinical effect of functional magnetic resonance imaging(fMRI)and angle prick method combined with meridian sinew row acupuncture in the treatment of refractory peripheral facial paralysis.Methods A total of 76 patients with refractory peripheral facial palsy were randomly divided into control group and observation group,with 38 cases in each group.They were treated with routine acupuncture,fMRI⁃based angular acupuncture combined with transsineural acupuncture for 4 weeks,respectively.The score of facial nerve function(Sunnybrook),facial nerve palsy symptom score and facial electromyography index were compared between the two groups,and the recovery rate was calculated.Results After treatment,Sunnybrook score in the observation group was higher than that in the control group(P<0.05),facial paralysis symptom score was lower than that in the control group(P<0.05),facial EMG blinking reflex latency,facial nerve conduction latency and wave amplitude parameters were better than those in the control group(P<0.05),and the recovery rate was higher than that in the control group(P<0.05).Conclusion The combination of fMRy⁃based angular acupuncture and transsineural acupuncture can effectively restore facial nerve function and improve clinical symptoms in patients with refractory peripheral facial palsy,and the clinical efficacy is superior.
