基于《针灸大成》论心身疾病辨治探赜

Discussion on the Psychosomatic Disease Treatment Based on the Great Compendium of Acupuncture and Moxibustion

目的挖掘《针灸大成》中有关心身疾病的相关论述,总结辨治规律。方法摘录《针灸大成》中符合心身疾病诊疗特点的原文,建立数据库,分析取穴规律。结果涉及心身疾病诊疗穴位共87个,共计频次140次;最常使用足太阳膀胱经,其次为任脉、足阳明胃经;最常使用的腧穴为足三里、中脘、肺俞、大陵;穴位主要集中在四肢部;所有穴位中包含70个特定穴,其中,五输穴使用频次最高,其次为交会穴和募穴。结论《针灸大成》治疗心身疾病具有取穴广泛,量少质佳,重用特定穴,多种配穴方法灵活应用,强调辨证论治,整体治疗,善用灸法的特点,为后世诊疗心身疾病提供了选穴思路与辨治要点。

Objective To investigate the relevant psychosomatic disease treatment based on the Great Compendium of Acupuncture and Moxibustion and to summarize the rules of treatment.Methods Comprehensive excerpts of the original text of the Great Compendium of Acupuncture and Moxibustion that meet the characteristics of the treatment of psychosomatic diseases,Excel software was used to build a database and analyze the laws of the meridians attribution,frequency,and specific points.Results There were 87 acupoints involved in the Great Compendium of Acupuncture and Moxibustion,and the frequency of acupoints was 140 times.The most commonly used meridians were the bladder meridian of foot-taiyang,ren meridian and the stomach meridian of foot-yangming.The most frequently used acupoints were Zusanli(ST36),Zhongwan(RN12),Feishu(BL13),Daling(PC7).Acupoints were mainly concentrated on the extremities.There were 70 specific acupoints accounted of the total selected acupoints.The most frequently used specific acupoints were five-shu points,crossing points and front-mu points.Conclusion The Great Compendium of Acupuncture and Moxibustion for the treatment of psychosomatic diseases is characterized by a wide range of acupuncture points,a small quantity of well quality,the use of specific acupuncture points,the flexible application of a variety of acupuncture point methods,the emphasis on evidence-based treatment,holistic treatment,and the good use of moxibustion,which has provided future generations with ideas for the selection of acupuncture points and the identification of treatment points for the treatment of psychosomatic diseases.
