郑氏手法结合针刺治疗中老年神经根型颈椎病对其颈椎生理曲度影响的临床观察

Clinical Observation on Effect of Zheng′s Manipulation Combined with Acupuncture on Physiological Curvature of Cervical Spine in Middle-Aged and Elderly Patients with Cervical Spondylotic Radiculopathy

目的观察郑氏手法结合针刺治疗中老年神经根型颈椎病的临床疗效及对其颈椎生理曲度的影响。方法将该院2021年6月—2022年5月收治的63例符合神经根型颈椎病急性期诊断标准的患者分为治疗组和对照组,其中治疗组在治疗中脱离2例,完成治疗30例,对照组在治疗中脱离1例,完成治疗30例。治疗组采用郑氏手法结合普通针刺治疗,对照组采用普通手法结合针刺治疗。1周治疗5次,两组均治疗2周。比较两组总体疗效,比较两组治疗前后视觉模拟评分法(visual analogue scale,VAS)、颈椎功能障碍指数(neck disability index,NDI)和颈椎病临床评价量表(clinical evaluation scale of cervical spondylosis,CASCS)评分变化;比较两组治疗前后颈椎生理曲度变化。结果治疗组治疗总有效率为86.67%(26/30),对照组治疗总有效率为63.33%(19/30)。治疗组总体疗效优于对照组(P<0.05);治疗组治疗后VAS评分、NDI评分低于对照组,CASCS评分高于对照组(P<0.05);治疗组治疗后颈椎生理弧度高于对照组(P<0.05)。结论郑氏手法结合针刺是治疗神经根型颈椎病的有效方案,值得临床进一步研究。

Objective To observe the clinical efficacy of Zheng′s manipulation combined with acupuncture in the treatment of middle-aged and elderly cervical spondylotic radiculopathy and its impact on the physiological curvature of the cervical spine.Methods Sixty patients who met the diagnostic criteria for the acute phase of cervical spondylotic radiculopathy admitted to the hospital between June 2021 and May 2022 were divided into a treatment group(30 cases)and a control group(30 cases).In the treatment group,2 cases broke away during the treatment and 30 cases completed the treatment,and 1 case in the control group broke away during the treatment and 30 cases completed the treatment.The treatment group was treated with Zheng′s manipulation combined with ordinary acupuncture,while the control group was treated with ordinary manipulation combined with acupuncture.The treatment was given 5 times a week,with both groups receiving the treatment for 2 weeks.The overall efficacy of the two groups was compared.The changes in visual analog scale(VAS),neck disability index(NDI)and clinical evaluation scale of cervical spondylosis(CASCS)scores before and after treatment were compared.The changes in cervical physiological curvature before and after treatment between the two groups were compared.Results In the treatment group,8 cases reached the cure standard,accounting for 26.67%;12 cases met the standard for significant efficacy,accounting for 40.00%;6 cases reached the effective standard,accounting for 20.00%.The total effective rate was 86.67%(26/30).In the control group,6 cases reached the cure standard,accounting for 20.00%;10 cases met the standard for significant efficacy,accounting for 33.33%.Three cases reached the effective standard,accounting for 10.00%.The total effective rate was 63.33%(19/30).The overall efficacy of the treatment group was better than that of the control group(P<0.05).After treatment,the VAS score and NDI score of the treatment group were lower than those of the control group,while the CASCS score was higher than that of the control group(P<0.05).After treatment,the physiological curvature of the cervical spine in the treatment group was higher than that in the control group(P<0.05).Conclusion Zheng′s manipulation combined with acupuncture can effectively relieve the clinical symptoms of patients with cervical spondylotic radiculopathy.It is a treatment plan worthy of further clinical research.
