经皮加压钢板与近端抗旋髓内钉治疗老年股骨粗隆间骨折的疗效与安全性

摘要 目的探讨经皮加压钢板(PCCP)与近端抗旋髓内钉(PFNA)治疗老年股骨粗隆间骨折的疗效与安全性。方法选取2019年10月至2021年2月九江市第六人民医院收治的70例老年股骨粗隆间骨折患者作为研究对象,随机分为对照组与研究组,每组35例,对照组行PCCP治疗,研究组行PFNA治疗。比较两组不同时间点Harris髋关节功能评分和术后并发症发生情况。结果两组Harris髋关节功能评分组间、时间比较差异有统计学意义(P<0.05);两组Harris髋关节功能评分交互比较差异无统计学意义。术后1周及术后2、6个月,两组Harris髋关节功能评分均高于前一时间点,且研究组高于对照组,差异有统计学意义(P<0.05)。研究组术后并发症发生率为8.58%,低于对照组的28.57%,差异有统计学意义(P<0.05)。结论老年股骨粗隆间骨折患者接受PFNA治疗后髋关节功能恢复情况良好,且安全性良好,值得临床推广应用。 Objective To compare the efficacy and safety of percutaneous compression plate(PCCP)and proximal femoral nail antirotation(PF-NA)in the treatment of elderly intertrochanteric fractures of femur.Methods A total of 70 elderly patients with intertrochanteric fracture of femur admitted to Jiujiang Sixth People's Hospital from October 2019 to February 2021 were selected as research subjects,and they were divided into the control group and the study group,with 35 cases in each group.The control group was treated with PCCP,and the study group was treated with PF-NA,the Harris hip function score at different time pointsand postoperative complications were compared between the two groups.Results There were significant differences in Harris hip function score between the two groups of group and time points(P<0.05);there was no significant differ-ence in Harris hip function between the two groups of interaction.At 1 week,2 and 6 months after surgery,the Harris hip function scores of the two groups were higher than the previous time point,and the study group was higher than the control group,and the difference was statistically signifi-cant(P<0.05).The incidence of postoperative complications was 8.58%in the study group,which was lower than 28.57%in the control group,and the difference was statistically significant(P<0.05).Conclusion The hip function of elderly patients with intertrochanteric fracture is well recov-ered after PFNA treatment,and it is safe and worthy of clinical promotion and application.
