机械通气下纤维支气管镜介入治疗重症肺部感染的临床疗效

Clinical effect of intervention treatment of severe lung infection with fibrobronchoscopic under mechanical ventilation

目的探究机械通气下纤维支气管镜介入治疗重症肺部感染的临床疗效。方法选取2019年1月至2020年12月福建医科大学附属闽东医院收治的230例重症肺部感染患者作为研究对象,按照随机数字表法分为对照组与观察组,每组115例。对照组采取对症治疗,观察组在对照组基础上联合纤维支气管镜介入治疗,比较两组机械通气指标、临床疗效及治疗前后炎症指标。结果观察组呼吸机撤机时间、体温恢复正常时间及咳痰消失所需时间均短于对照组,差异有统计学意义(P<0.05)。观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,两组WBC、中性粒细胞计数及降钙素原水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。结论纤维支气管镜介入治疗重症肺部感染疗效确切,可缩短患者恢复时间,降低炎症因子水平,值得临床推广应用。

Objective To explore the clinical effect of intervention treatment of severe lung infection with fibrobronchoscopic under mechanical ventilation.Methods 230 patients with severe lung infection admitted to Mindong Hospital Affiliated to Fujian Medical University from January 2019 to December 2020 were selected as the study subjects,and they were divided into the control group and the observation group according to ran-dom number table method,with 115 cases in each group.The control group was treated with symptomatic treatment,and the observation group was combined with fibrobronchoscopy interventional treatment on the basis of the control group,the indexes of mechanical ventilation,clinical efficacy and inflammation before and after treatment were compared between the two groups.Results The time of ventilator withdrawal,temperature return to normal and sputum disappearance in the observation group were shorter than those in the control group,the differences were statistically signifi-cant(P<0.05).The total effective rate in the observation group was higher than that in the control group,and the difference was statistically signifi-cant(P<0.05).After treatment,the WBC,neutrophil count and raw calcitonin water of the two groups were lower than before treatment,and the ob-servation group was lower than the control group,the differences were statistically significant(P<0.05).Conclusion Fibrobronchoscopy interven-tion in the treatment of severe pulmonary infection is effective,can shorten the recovery time of patients,reduce the level of inflammatory factors,and is worthy of clinical promotion and application.
