老年慢性心力衰竭患者低蛋白血症现状及影响因素分析

Current status and influencing factors of hypoproteinemia in elderly patients with chronic heart failure

目的分析老年慢性心力衰竭(CHF)患者低蛋白血症现状及其影响因素。方法选取207例老年CHF患者作为研究对象,根据患者入院24 h内血清总蛋白情况,将其分为低蛋白血症组(n=58)和非低蛋白血症组(n=149)。比较两组患者一般资料和临床资料,应用logistic回归模型分析老年CHF患者发生低蛋白血症的影响因素。结果老年CHF患者低蛋白血症的发生率为28.02%。logistic回归模型分析结果表明,有原发冠心病(OR=3.292)、血清白蛋白<35 g/L(OR=7.597)和血清钙<2 mmol/L(OR=9.603)均是老年CHF患者发生低蛋白血症的危险因素(均P<0.05)。结论老年CHF患者常合并低蛋白血症,原发冠心病、血清白蛋白和血清钙水平低均是老年CHF患者发生低蛋白血症的危险因素。

Objective To analyze the current status and influencing factors of hypoproteinemia in elderly patients with chronic heart failure(CHF).Methods A total of 207 elderly patients with CHF were selected as the study subjects and divided into the hypoproteinemia group(n=58)or the non-hypoproteinemia group(n=149)according to their serum total protein levels within 24 hours of admission.The general and clinical data of the two groups were compared,and the logistic regression model was used to analyze influencing factors of hypoproteinemia in elderly patients with CHF.Results The incidence of hypoproteinemia in elderly patients with CHF was 28.02%.The results of logistic regression model analysis showed that the presence of primary coronary heart disease(OR=3.292),serum albumin level<35 g/L(OR=7.597),and serum calcium level<2 mmol/L(OR=9.603)were risk factors of hypoproteinemia in elderly patients with CHF(all P<0.05).Conclusion Elderly patients with CHF are usually complicated with hypoproteinemia,and primary coronary heart disease,low serum albumin and calcium levels are risk factors of hypoproteinemia in elderly CHF patients.
