医生-护士-患者三位一体工作模式在患者全程化管理中的应用探索

目的 探讨医生-护士-患者三位一体工作模式在患者全程化管理中的应用效果。方法 构建医生-护士-患者三位一体工作模式,并应用于患者全程化管理中,实施前选取医生27人、护士60人及2021-06~07月于作者科室就诊的382例患者作为对照组,实施后选取原有科室医生27人、护士60人及2022-02~03月于作者科室就诊的382例患者作为观察组。通过发放问卷的形式调查两组在医护合作度、责任护士病情知晓率、患者满意度、随访率、复诊率方面的情况。结果 观察组医护合作度、责任护士病情知晓率、患者满意度、随访率、复诊率均优于对照组,组间比较差异具有统计学意义(P均<0.05)。结论 医生-护士-患者三位一体工作模式在患者全程化管理中的实施与探讨中取得了显著成效,对提高医院整体的医疗服务质量价值显著,值得推广应用。

Objective To explore the application of doctor-nurse-patient integration model in whole-process management of patients.Methods Doctor-nurse-patient integration model was established and applied to whole-process management of patients;before the implementation,27 doctors,60 nurses and 382 patients who were admitted to the author′s department from June to July 2021 were selected as the control group;after the implementation,27 doctors and 60 nurses in the original department and 382 patients admitted to the author′s department from February to March 2022 were selected as the observation group.The two groups were investigated and compared in terms of the degree of medical cooperation,disease awareness rate of the responsible nurses,the satisfaction of patients,the follow-up rate and the return visit rate by questionnaires.Results The degree of cooperation between doctors and nurses,disease awareness rate of the responsible nurses,the satisfaction of patients,the follow-up rate and the return visit rate in the observation group were better than those in the control group,and the differences between the two groups were statistically significant(all P<0.05).Conclusion The implementation and exploration of the doctor-nurse-patient integration work mode in the whole process of patient management has achieved remarkable results,which has significant value in improving the overall quality of medical services of the hospital,and is worthy of popularization and application.
