摘要
目的评估杭州市健康人群白喉抗体水平。方法采用分层抽样方法于2018-2021年选择杭州市4个区健康人群采集血清标本,采用酶联免疫吸附试验检测白喉IgG抗体,分析抗体阳性率和几何平均浓度(Geometric mean concentration,GMC)。结果调查对象白喉IgG抗体总阳性率为79.74%(1086/1362),GMC为0.49IU/mL。0-2月龄、3-11月龄、1-2岁、3-5岁、6-12岁、13-19岁、20-39岁、≥40岁人群抗体阳性率分别为77.36%、93.18%、97.16%、95.02%、92.65%、61.38%、67.46%、42.37%;GMC分别为0.33、0.65、0.70、0.67、0.72、0.28、0.22、0.15IU/mL。多因素分析显示,年龄、地区、含白喉成分疫苗(Diphtheria-containing vaccines,DCV)接种史和接种后时间是抗体阳性率和GMC的影响因素。结论杭州市健康人群白喉抗体水平总体较高,但随着年龄增长或DCV接种后时间延长而下降。需探讨青少年和成人白喉免疫策略。
Objective To evaluate diphtheria antibody levels in a healthy population in Hangzhou city.Methods We used a stratified sampling method to recruit healthy people in 4 districts of Hangzhou during 2018-2021 and obtained serum samples from subjects.We tested sera for diphtheria IgG antibody using enzyme-linked immunosorbent assay to determine positivity rates and geometric mean concentrations(GMC).Results The overall positivity rate of diphtheria IgG antibody was 79.74%(1086/1362),and the GMC was 0.49 IU/mL.Positivity rates for 0-2-month-olds,3-11-month-olds,1-2-year-olds,3-5-year-olds,6-12-year-olds,13-19-year-olds,20-39-year-olds,and≥40-year-olds were 77.36%,93.18%,97.16%,95.02%,92.65%,61.38%,67.46%,and 42.37%;respective GMCs were 0.33,0.65,0.70,0.67,0.72,0.28,0.22,and 0.15IU/mL.Multivariate analyses showed that age,region,diphtheria vaccination history,and time since vaccination were factors influencing positivity rate and GMC.Conclusions The diphtheria IgG antibody level was high among healthy people in Hangzhou but decreased with increasing age and time since diphtheria vaccination.We need to explore diphtheria immunization strategies for adolescents and adults.
