双源CT灌注成像参数对非小细胞肺癌患者化疗效果的预测价值 被引量:1

摘要 目的:探讨双源CT灌注成像参数对非小细胞肺癌患者化疗效果的预测价值。方法:回顾性分析2020年6月至2022年6月该院收治的124例非小细胞肺癌患者的临床资料,入院后均给予化疗治疗,治疗前后分别进行双源CT灌注成像检查,根据化疗效果分为敏感组(n=81)和不敏感组(n=43),比较两组治疗前后双源CT灌注成像参数[血流量(BF)、血容量(BV)、表面通透性(PS)],并绘制受试者工作特征(ROC)曲线,分析双源CT灌注成像参数对非小细胞肺癌患者化疗效果的预测价值。结果:治疗前,敏感组BF、PS、BV均高于不敏感组,差异有统计学意义(P<0.05);治疗后,两组BF、PS、BV均低于治疗前,且敏感组低于不敏感组,差异有统计学意义(P<0.05);ROC曲线结果显示,BF、PS、BV单项及联合检测预测非小细胞肺癌患者化疗效果的曲线下面积分别为0.767、0.824、0.803、0.932,联合检测的预测价值最高。结论:双源CT灌注成像参数联合检测预测非小细胞肺癌患者化疗效果的价值高于三者单项检测。 Objective:To investigate predictive value of dual-source CT perfusion imaging parameters for chemotherapy effects in patients with non-small cell lung cancer.Methods:The clinical data of 124 patients with non-small cell lung cancer admitted to this hospital from June 2020 to June 2022 were retrospectively analyzed.All patients were given chemotherapy after admission.Dual-source CT perfusion imaging was performed before and after the treatment.According to the chemotherapy effects,they were divided into sensitive group(n=81)and insensitive group(n=43).The dual-source CT perfusion imaging parameters[blood flow(BF),blood volume(BV),permeability surface(PS)]were compared between the two groups before and after the treatment.The receiver operating characteristic(ROC)curve was drawn to analyze the predictive value of dual-source CT perfusion imaging parameters for chemotherapy effects in the patients with non-small cell lung cancer.Results:Before the treatment,the BF,PS and BV of the sensitive group were higher than those of the insensitive group,and the differences were statistically significant(P<0.05).After the treatment,the BF,PS and BV of the two groups were lower than those before the treatment,those in the sensitive group were lower than those in the insensitive group,and the differences were statistically significant(P<0.05).The ROC curve showed that the areas under the curve of single detection of BF,PS,BV and combined detection in the prediction of the chemotherapy effects of the non-small cell lung cancer patients was 0.767,0.824,0.803 and 0.932,separately,and the combined detection had the highest predictive value.Conclusions:The value of combined detection of dual-source CT perfusion imaging parameters in the prediction of the chemotherapy effects in the patients with non-small cell lung cancer is higher than that of single detection of the three.
