血清25(OH)D、AMH、β-hCG水平联合检测对不孕症患者体外受精-胚胎移植妊娠失败的预测价值

摘要 目的:分析血清25羟维生素D[25(OH)D]、抗米勒管激素(AMH)、β-人绒毛膜促性腺激素(β-hCG)水平联合检测对不孕症患者体外受精-胚胎移植(IVF-ET)妊娠失败的预测价值。方法:回顾性分析2021年2月至2023年2月于该院接受IVF-ET的158例不孕症患者的临床资料,根据妊娠结局将其分为妊娠成功组(n=91)和妊娠失败组(n=67)。比较两组胚胎移植日血清25(OH)D、AMH、β-hCG水平,分析其与不孕症患者IVF-ET妊娠失败的相关性,并绘制受试者工作特征(ROC)曲线分析其上述指标单项及联合检测对不孕症患者IVF-ET妊娠失败的预测价值。结果:妊娠失败组胚胎移植日血清25(OH)D、AMH、β-hCG水平均低于妊娠成功组,差异有统计学意义(P<0.05);Pearson相关性分析结果显示,胚胎移植日血清25(OH)D、AMH、β-hCG水平与不孕症患者IVF-ET妊娠失败均呈负相关(r<0,P<0.05);ROC曲线结果显示,胚胎移植日血清25(OH)D、AMH、β-hCG水平单项及联合检测预测不孕症患者IVF-ET妊娠失败的曲线下面积分别为0.746、0.753、0.817、0.913,联合检测的预测价值最高。结论:血清25(OH)D、AMH、β-hCG水平联合检测对不孕症患者IVF-ET妊娠失败的预测价值高于三者单项检测。 Objective:To analyze predictive value of combined detection of serum 25-hydroxyvitamin D[25(OH)D],anti-Müllerian hormone(AMH)andβ-human chorionic gonadotropin(β-hCG)levels on pregnancy failure of in vitro fertilization-embryo transfer(IVF-ET)in infertile patients.Methods:The clinical data of 158 infertile patients who received IVF-ET in this hospital from February 2021 to February 2023 were retrospectively analyzed.According to the pregnancy outcome,these patients were divided into pregnancy successful group(n=91)and pregnancy failure group(n=67).The levels of serum 25(OH)D,AMH andβ-hCG on the day of embryo transfer were compared between the two groups,and their correlations with IVF-ET pregnancy failure were analyzed.The receiver operating characteristic(ROC)curve was drawn to analyze the predictive value of single and combined detection for IVF-ET pregnancy failure in the infertile patients.Results:The levels of serum 25(OH)D,AMH andβ-hCG on the day of embryo transfer in the pregnancy failure group were lower than those in the pregnancy successful group,and the differences were statistically significant(P<0.05).Pearson correlation analysis showed that serum 25(OH)D,AMH andβ-hCG levels on the day of embryo transfer were negatively correlated with IVF-ET pregnancy failure in the infertile patients(r<0,P<0.05).The ROC curve showed that the area under the curve of single and combined detection of serum 25(OH)D,AMH andβ-hCG levels on the day of embryo transfer for predicting IVF-ET pregnancy failure in the infertile patients was 0.746,0.753,0.817 and 0.913,respectively,and the combined detection had the highest predictive value.Conclusions:The predictive value of combined detection of serum 25(OH)D,AMH andβ-hCG levels for IVF-ET pregnancy failure in the infertile patients is higher than that of single detection of the three.
