血清胰岛素样生长因子-1、β-联蛋白、性激素、骨转换生化标志物水平与绝经后骨质疏松症患者骨密度的相关性

摘要 目的:观察血清胰岛素样生长因子-1(IGF-1)、β-联蛋白、性激素、骨转换生化标志物水平与绝经后骨质疏松症患者骨密度的相关性。方法:选取2018年8月至2020年3月该院收治的82例绝经后骨质疏松症患者进行横断面研究,设为骨质疏松组;选取同期于该院进行体检的60例绝经后未发生骨质疏松症女性进行横断面研究,设为对照组。比较两组间、骨质疏松组内骨折与非骨折患者性激素[雌二醇、促黄体生成激素(LH)、促卵泡成熟激素(FSH)、孕酮]水平、骨密度水平、IGF-1水平、β-联蛋白水平、骨转换生化标志物[Ⅰ型前胶原氨基端延长肽(PINP)、β-胶原降解产物(β-CTX)、骨钙素N端中分子(N-BGP)]水平;采用Pearson相关性分析IGF-1、β-联蛋白、性激素、骨转换生化标志物水平与绝经后骨质疏松症患者骨密度的相关性进行分析。结果:骨质疏松组雌二醇、孕酮、骨密度、IGF-1、β-联蛋白水平均低于对照组,LH、FSH、PINP、β-CTX和N-BGP水平均高于对照组,差异有统计学意义(P<0.05);骨质疏松组骨折发生率为36.59%(30/82);骨质疏松组骨折患者雌二醇、孕酮、骨密度、IGF-1、β-联蛋白水平明显低于非骨折患者,LH、FSH、PINP、β-CTX和N-BGP水平明显高于非骨折患者,差异均有统计学意义(P<0.05);Pearson相关性分析结果显示,绝经后骨质疏松症患者血清雌二醇、孕酮、IGF-1、β-联蛋白水平均与骨密度均呈正相关(r>0,P<0.05),LH、FSH、PINP、β-CTX、N-BGP水平与骨密度均呈负相关(r<0,P<0.05)。结论:血清雌二醇、孕酮、IGF-1、β-联蛋白水平与绝经后骨质疏松症患者骨密度均呈正相关,LH、FSH、PINP、β-CTX、N-BGP水平与骨密度均呈负相关。 Objective:To observe correlations among serum insulin-like growth factor-1(IGF-1),β-catenin,sex hormones,bone turnover biochemical marker levels and bone mineral density in patients with postmenopausal osteoporosis.Methods:A cross-sectional study was conducted on 82 patients with postmenopausal osteoporosis admitted to this hospital from August 2018 to March 2020,and they were set as osteoporosis group.60 postmenopausal women without osteoporosis who underwent physical examination in the hospital during the same period were selected for the cross-sectional study,and they were set as the control group.The levels of sex hormones[estradiol,luteinizing hormone(LH),follicle stimulating hormone(FSH),progesterone],bone mineral density,IGF-1,β-catenin,bone turnover biochemical markers[N-terminal propeptide of type 1 procollagen(PINP),β-collagen degradation product(β-CTX),N-terminal molecules of osteocalcin(N-BGP)]were compared between the two groups and among the patients with fracture and without fracture in the osteoporosis group.Pearson correlation analysis was used to analyze the correlations among the IGF-1,β-catenin,sex hormones,bone turnover biochemical marker levels and bone mineral density in the patients with postmenopausal osteoporosis.Results:The levels of estradiol,progesterone,bone mineral density,IGF-1 andβ-catenin in the osteoporosis group were lower than those in the control group;the levels of LH,FSH,PINP,β-CTX and N-BGP were higher than those in the control group;and the differences were statistically significant(P<0.05).The incidence of fracture in the osteoporosis group was 36.59%(30/82).The levels of estradiol,progesterone,bone mineral density,serum IGF-1 and serumβ-catenin in the fracture patients of osteoporosis group were significantly lower than those in the nonfracture patients,the levels of LH,FSH,PINP,β-CTX and N-BGP were significantly higher than those in the non-fracture patients,and the differences were statistically significant(P<0.05).Pearson correlation analysis showed that the serum estradiol,progesterone,IGF-1 andβ-catenin levels in the postmenopausal osteoporosis patients were positively correlated with bone mineral density(r>0,P<0.05);and the levels of LH,FSH,PINP,β-CTX and N-BGP were negatively correlated with bone mineral density(r<0,P<0.05).Conclusions:The levels of serum estradiol,progesterone,IGF-1 andβ-catenin are positively correlated with bone mineral density in the patients with postmenopausal osteoporosis,and the levels of LH,FSH,PINP,β-CTX and N-BGP are negatively correlated with bone mineral density.
