LncRNA PVT1在非小细胞肺癌中的表达及其与脑转移的相关性

摘要 目的探讨长非编码RNA(lncRNA)浆细胞瘤变异易位1(PVT1)在非小细胞肺癌(NSCLC)患者中的表达情况,并分析其与脑转移的关系。方法选取2018年1月至2022年6月在南京市溧水区人民医院就诊并确诊为NSCLC患者95例为研究对象,根据是否发生脑转移,分为发生脑转移组(n=40)和未发生脑转移组(n=55);随机选取同期健康人群50例为对照组。收集临床资料,采用实时荧光定量反转录聚合酶链反应(qRT-PCR)检测血清及癌组织中lncRNA PVT1水平,Spearman法分析血清lncRNA PVT1表达水平与脑转移发生的相关性;采用受试者工作特征(ROC)曲线评价血清lncRNA PVT1水平对NSCLC患者发生脑转移的诊断价值;采用多因素Logistic回归分析NSCLC患者发生脑转移的影响因素。结果与未发生脑转移组比较,发生脑转移组患者血清及癌组织中lncRNA PVT1水平升高,差异均有统计学意义(P<0.05);Spearman相关性分析结果显示,血清中lncRNA PVT1水平与NSCLC患者脑转移呈正相关(r=0.618,P<0.05);ROC曲线结果显示,血清lncRNA PVT1水平诊断诊断NSCLC患者发生脑转移的曲线下面积为0.859(95%CI 0.773~0.922),灵敏度为80.00%,特异度为87.27%;多因素Logistic回归分析结果显示,lncRNA PVT1、临床分期、淋巴结转移、分化程度均与NSCLC患者发生脑转移有关(P<0.05)。结论发生脑转移的NSCLC患者血清及癌组织中lncRNA PVT1表达水平升高,检测血清lncRNA PVT1表达水平可用于诊断NSCLC患者脑转移的发生。 Objective To investigate the expression of long non-coding RNA(lncRNA)plasmacytoma variant translocation 1(PVT1)in patients with non-small cell lung cancer(NSCLC),and to analyze its relationship with brain metastasis.Methods Ninety-five patients diagnosed with NSCLC who were treated in Lishui District People’s Hospital of Nanjing from January,2018 to June,2022 were selected as the study subjects.According to whether they had BMS,these patients were divided into two groups:BMS group(n=40)and no BMS group(n=55).Fifty healthy people were randomly selected as the control group.According to whether they had BMS,these subjects were divided into the group with BMS(n=40)and the group without BMS(n=55).Clinical data of patients in 2 groups were collected,and lncRNA PVT1 levels in serum and cancer tissues were detected by real-time quantitative fluorescence reverse transcription polymerase chain reaction(qRT-PCR).Spearman method was performed to analyze the correlation between the expression level of serum lncRNA PVT1 and the occurrence of brain metastasis;Receiver operating characteristic(ROC)curve was drawn to evaluate the diagnostic value of serum lncRNA PVT1 level for brain metastasis in NSCLC patients;Multivariate logistic regression analysis was performed to analyze the influencing factors of brain metastasis in NSCLC patients.Results Compared with the no brain metastasis group,the level of lncRNA PVT1 in the serum and cancer tissue of the brain metastasis group was greatly increased(P<0.05);Spearman correlation analysis showed that the level of lncRNA PVT1 in serum was positively correlated with brain metastasis in NSCLC patients(r=0.618,P<0.05);ROC curve showed that the area under the curve(AUC)of serum lncRNA PVT1 level to predict BMS in patients diagnosed with NSCLC was 0.859(95%CI 0.773-0.922),with a sensitivity of 80.00%and specificity of 87.27%;Multivariate logistic regression analysis showed that lncRNA PVT1,clinical stage,lymph node metastasis,and degree of differentiation were all associated with brain metastasis in NSCLC patients(P<0.05).Conclusion The expression level of lncRNA PVT1 in serum and cancer tissue of NSCLC patients with brain metastasis is increased.Detection of serum lncRNA PVT1 expression level can be used to diagnose the occurrence of brain metastasis in NSCLC patients.
