摘要
目的探讨血清胃功能3项指标检测对于幽门螺杆菌(Hp)感染相关胃癌早期诊断价值。方法选择2021年2月至2023年4月消化内科病房接收的病理确诊胃癌患者80例为胃癌组,同期接收的80例慢性胃炎患者为胃炎组。胃癌组同时根据分期分为早期胃癌组与进展期胃癌组。在胃癌组和胃炎组中对PG I、PG II和PG I/PGII 3项指标进行比较,同时对早期胃癌与进展期胃癌检测结果进行比对。经14 C尿素呼气试验判定胃癌组Hp感染阳性情况,对Hp阳性与阴性患者胃功能3项测定结果进行比较。结果胃癌组PG I、PG I/PG II低于胃炎组PG II高于胃炎组(P<0.05);早期胃癌PG I、PG I/PG II高于进展期胃癌(P<0.05),早期与进展期胃癌的PG II差异无统计意义(P>0.05);14 C尿素呼气试验证实Hp阳性47例(58.75%),胃癌组Hp阳性患者PG I、PG I/PG II低于Hp阴性患者,PG II高于Hp阴性患者(P<0.05)。结论血清PG I、PG II、PG I/PG II联合检测,能够早期识别胃癌,并且评估病情进展情况,这为病变早期干预提供重要帮助。
Objective To evaluate the value of three indexes of serum gastric function in the early diagnosis of gastric cancer associated with Helicobacter pylori(Hp)infection.Methods 80 patients with pathologically confirmed gastric cancer admitted to the Gastroenterology Ward from February,2021 to April,2023 were selected as the gastric cancer group,while 80 patients with chronic gastritis were selected as the gastritis group during the same period.The gastric cancer group was divided into the early gastric cancer and advanced gastric cancer groups.PGI,PGII and PGI/PGII were compared between the gastric cancer group and gastritis group,and the detection results of the early gastric cancer and advanced gastric cancer were compared.The positive Hp infection was determined by 14 C urea breath test,and the gastric function results of Hp positive and negative patients were compared.Results PGI and PGI/PGII in the gastric cancer group were lower than those in the gastritis group,and PGII was higher than those in the gastritis group(P<0.05).PG and PGI/PGII in the early gastric cancer group were higher than those in the advanced gastric cancer group(P<0.05),but there was no significant difference in PGII between the early and advanced gastric cancer groups(P>0.05).The 14 C urea breath test confirmed that 47 cases(58.75%)were Hp positive.PGI and PGI/PGII in HP-positive patients in the gastric cancer group were lower than those in HP-negative patients,and PGII was higher than those in HP-negative patients(P<0.05).Conclusion Serum PGI,PGII,PGI/PGII combined detection can identify gastric cancer early and evaluate the progression of the disease,which provides an important reference for the early intervention of lesions.
