摘要
皮肤是人体最大的免疫器官,是外界环境因素作用于机体的第一道防线。日晒是多种常见皮肤病的诱因,日光中的紫外线可通过影响皮肤固有免疫系统,一方面参与各种日光性皮肤病的发生、发展;另一方面被应用于各种皮肤疾病的治疗。本文通过阐述紫外线对皮肤固有免疫系统各成分的影响,如急性紫外线暴露下角质形成细胞、真皮树突状细胞以及组织驻留记忆T细胞导致的免疫激活反应,以及慢性紫外线暴露下朗格汉斯细胞、肥大细胞、先天淋巴样细胞等参与的免疫抑制反应,以期为临床上更好理解日光性皮肤病的发病机制及光疗选择提供思路。
Skin is the body′s largest immune organ that functions as the first site of defense to external environmental factors.Sunlight exposure can give rise to numerous common skin diseases,exhibiting a dual role in dermatology.On one hand,ultraviolet radiation(UVR) inherent in sunlight exerts a significant impact on the skin′s innate immunity,thereby contributing to the development and progression of various photodermatoses.On the other hand,UVR is also harnessed for therapeutic purposes in the treatment of various skin conditions.This paper delves into an exploration of the diverse effects of ultraviolet light on the different components of the skin′s innate immune system.It elucidates the immune activation responses elicited by keratinocytes,dermal dendritic cells,and tissue-resident memory T cells following acute exposure to ultraviolet light.Additionally,it investigates the immunosuppressive reactions involving Langerhans cells,mast cells,and innate lymphoid cells under conditions of chronic ultraviolet light exposure.The aim of this comprehensive discussion is to enhance our understanding of the underlying pathogenesis of photodermatoses and facilitate informed decision-making in the context of phototherapy selection.
