基于PDCA模式的镇痛镇静护理对ICU患者护理质量影响观察

Effect of PDCA-based Analgesic and Sedative Nursing on Nursing Quality of ICU Patients

目的 分析基于PDCA模式的镇痛镇静护理对ICU患者护理质量影响。方法 本次研究总计纳入2020年01月-2022年12月收治的99例ICU患者,将患者以随机数字表法分为干预组(n=49例)与参照组(n=50例),干预组患者实施基于PDCA模式的镇痛镇静护理,参照组患者实施常规护理干预,各组护理结局比较:采取BPS疼痛评分量表及Ramsay镇静评分量表评估各组患者镇痛及镇静效果、各组住院指标(镇静时间以及镇静药物用量、ICU治疗时间与总住院时间)、并发症、干预前后Richards Campbell睡眠量表(RCSQ)评分变化、满意率。结果 各组干预前BPS疼痛评分及Ramsay镇静评分、RCSQ评分比较,P均>0.05,各组干预后患者BPS疼痛评分及Ramsay镇静评分、RCSQ评分均改善,干预组BPS疼痛评分及Ramsay镇静评分、RCSQ评分均优于参照组,P<0.05;干预组患者各组住院指标(镇静时间以及镇静药物用量、ICU治疗时间与总住院时间)均优于参照组,P<0.05;干预组患者并发症率比参照组低,干预满意率比参照组高,P均<0.05。结论 基于PDCA模式的镇痛镇静护理可较好促进ICU患者镇静镇痛,患者恢复较好,并发症少,睡眠好,满意率高,可进行应用。

Objective To analyze the effect of PDCA-based analgesic and sedative nursing on ICU patients′nursing quality.Methods A total of 99 ICU patients(admitted from January 2020 to December 2022)were included in this study.The patients were divided into the intervention group(n=49 cases)and the reference group(n=50 cases)by random number table method.The patients in the intervention group received analgesic sedation nursing based on PDCA mode,and the patients in the reference group received routine nursing intervention.BPS pain rating scale and Ramsay sedation rating scale were used to evaluate analgesia and sedation effects in each group,hospitalization indicators(sedation time and dosage,ICU treatment time and total length of stay),complications,Richards Campbell Sleep Scale(RCSQ)score changes before and after intervention,and satisfaction rate.Results Comparison of BPS pain score,Ramsay sedation score and RCSQ score before intervention in each group showed that P>0.05.After intervention,BPS pain score,Ramsay sedation score and RCSQ score were improved in each group.BPS pain score,Ramsay sedation score and RCSQ score in the intervention group were better than those in the reference group.P<0.05;Hospitalization indexes(duration of sedation,dosage of sedative drugs,duration of ICU treatment and total length of stay)in intervention group were better than those in reference group,P<0.05.The complication rate of the intervention group was lower than that of the reference group,and the intervention satisfaction rate was higher than that of the reference group,all P<0.05.Conclusions Analgesic and sedative nursing based on PDCA model can better promote sedation and analgesia in ICU patients,patients recover better,less complications,good sleep,high satisfaction rate,and can be applied.
