局麻下经超声引导行真空辅助乳腺微创旋切治疗男性乳腺发育症的护理体会

Nursing Experience for Patients with Gynaecomastia Undergoing Vacuum-assisted Breast Biopsy Treatment Under Local Anesthesia

目的:总结局麻下经超声引导行真空辅助乳腺微创旋切治疗男性乳腺发育症(GYN)的护理经验,为临床护理提供依据。方法:回顾性分析2018年11月—2021年10月重庆市妇幼保健院收治的4例GYN患者的病历资料,对4例GYN患者在局麻下经超声引导行真空辅助乳腺微创旋切治疗,术前充分准备与评估,术中人文关怀与疼痛护理,术后对并发症及医源性损伤及时观察预防,同时注重围手术期的心理护理与隐私保护,做好出院准备度评估与出院指导。结果:术后4例患者均未发生出血、皮下积液、感染坏死等并发症及医源性皮肤损伤,术后美容效果评分高,乳头敏感度高,术后2~4 d均病情稳定出院。结论:对局麻下经超声引导行真空辅助乳腺微创旋切治疗的GYN患者实施围手术期护理,强化疼痛管理与心理护理,注重隐私保护和人文关怀,及时识别处理并发症及医源性损伤,可为手术治疗与美容效果提供有力保障。

Objective:To summarize the nursing experience of vacuum-assisted minimally invasive breast spinning under local anesthesia via ultrasound guidance for the treatment of gynecomastia(GYN),and to provide a basis for clinical care.Methods:The medical records of 4 patients with GYN admitted to the hospital from November 2018 to October 2021 were retrospectively analyzed,and 4 patients with GYN were treated with vacuum-assisted minimally invasive breast rotational excision under local anesthesia with ultrasound guidance.Patients were adequately prepared and evaluated preoperatively,intraoperative humanistic care and pain care were provided,postoperative complications and medically induced injuries were observed and prevented in a timely manner,while perioperative psychological care and privacy protection were emphasized,and discharge readiness was assessed and discharge instructions were given.Results:None of the four postoperative patients had complications such as bleeding,subcutaneous effusion,infection necrosis,or medical skin injury,the postoperative cosmetic effect scores were high,nipple sensitivity was high,and they were discharged from the hospital with stable conditions 2 to 4 d after surgery.Conclusion:Perioperative nursing care for GYN patients treated with vacuum-assisted minimally invasive breast rotary excision under local anesthesia via ultrasound guidance,strengthening pain management and psychological care,focusing on privacy protection and humanistic care,and timely identification and treatment of complications and medical source injuries can provide strong guarantee for surgical treatment and cosmetic results.
