穴位敷贴联合集束化护理对ICU机械通气胃肠功能障碍患者的影响

Effect of Acupoint Application Combined with Cluster Nursing on Patients with Gastrointestinal Dysfunction during Mechanical Ventilation in ICU

目的观察穴位敷贴联合集束化护理对重症加强护理病房(ICU)机械通气胃肠功能障碍(GID)患者的影响。方法选择泉州市第一医院ICU收治的64例GID患者,按照随机数字表法分为对照组和试验组,各32例。对照组在常规治疗和护理基础上接受集束化护理,试验组在对照组基础上接受穴位敷贴治疗;分别于治疗前后评价两组患者胃肠功能障碍与衰竭(GIDF)评分、胃肠功能恢复情况、腹围和急性生理与慢性健康状况Ⅱ(APACHEⅡ)评分变化。结果治疗后,两组GIDF评分,首次肛门排气、首次排便、肠鸣音恢复、进食恢复时间,腹围和APACHEⅡ评分均较治疗前明显降低,且试验组低于对照组(P<0.05)。结论穴位敷贴联合集束化护理可改善GID患者胃肠功能,促进排气、排便和身体健康恢复,值得临床推广。

Objective To observe the effect of acupoint application combined with cluster nursing on patients with gastrointestinal dysfunction(GID)during mechanical ventilation in ICU.Methods A total of 64 patients with GID in ICU of Quanzhou First Hospital were randomly divided into control group and experiment group,with 32 patients in each group.The control group received cluster nursing on the basis of routine treatment and nursing.The experiment group received acupoint application treatment on the basis of the control group,once a day,7 days per week,continuous treatment for one week.Gastrointestinal dysfunction and failure(GIDF)scores of GID patients,the recovery of gastrointestinal function,changes in abdominal circumference and acute physiology and chronic health evaluationⅡ(APACHEⅡ)score were evaluated before and after treatment.Results After treatment,GIDF scores,first anal exhaust,first defecation,bowel sound recovery,resumption of eating time,abdominal circumference and APACHEⅡscores in both groups were significantly lower than before treatment,and the test group was lower than the control group(P<0.05).Conclusion Acupoint application combined with cluster nursing can improve gastrointestinal function of GID patients,promote exhaust,defecation and physical health recovery,which is worthy of clinical promotion.
