江苏启东地区乙型肝炎疫苗接种人群HBV感染与ABO血型的关系

Relationship of HBV infection and ABO blood group in the population underwent hepatitis B vaccine in Qidong area

目的分析江苏启东地区乙型肝炎(乙肝)疫苗接种人群HBV感染与ABO血型的关系。方法2018—2020年随机抽取江苏启东市乙肝干预研究队列中的12214例为研究对象。其中,新生儿期接种乙肝疫苗6595例(疫苗组),未接种乙肝疫苗5619例(对照组)。检测ABO血型、HBsAg、HBV表面抗体(抗-HBs)和HBV核心抗体(抗-HBc),分析不同血型与HBV感染之间的关系。结果12214例中,AB型1122例,A型3585例,B型3466例,O型4041例。疫苗组HBsAg和抗-HBc阳性率分布为1.79%和4.76%,低于对照组的7.54%和23.88%(P<0.01)。疫苗组中,AB型和A型血HBsAg阳性率分别为2.69%和2.30%,高于O型血的1.34%(P<0.05);与O型血比较,AB型血HBsAg阳性风险增加了1.01倍(P<0.05),A型血HBsAg阳性风险增加了72%(P<0.05)。结论江苏启东地区乙肝疫苗接种人群发生HBV感染与ABO血型有关,AB型和A型血人群发生HBV感染的风险高于O型血。

Objective To analyze the relationship of HBV infection and ABO blood group in the population underwent hepatitis B vaccine in Qidong area of Jiangsu Province.Methods A total of 12214subjects were randomly selected from Qidong Hepatitis B Intervention Study cohort in 2018—2020,of whom 6595cases(group A)accepted hepatitis B vaccine and 5619cases(group B)did not accept hepatitis B vaccine during neonatal period.ABO blood groups,HBsAg,HBV surface antibodies(anti-HBs)and HBV core antibodies(anti-HBc)were detected.The relationship between ABO blood groups and HBV infection was analyzed.Results In 12214subjects,1122cases were with blood type AB,3585cases were with blood type A,3466cases were with blood type B,and 4041cases were with blood type O.The positive rates of HBsAg and anti-HBc were lower in group A than those in group B(1.79%and 4.76%vs.7.54%and 23.88%)(P<0.01).The positive rate of HBsAg was higher in the subjects with blood type AB and A than those with blood type O in group A(2.69%and 2.30%vs.1.34%)(P<0.05).Compared with the subjects with blood type O,the risk of HBsAg positivity was increased 1.01fold in the subjects with blood type AB and that was increased 72%in the subjects with blood type A in group A(P<0.05).Conclusion ABO blood group is related to the HBV infection in the population underwent hepatitis B vaccine in Qidong area of Jiangsu Province.The risk of HBV infection is higher in the population with blood type AB and A than that in those with blood type O.
