腹腔镜阑尾切除术中应用双极电凝法处理阑尾根部及系膜的临床疗效分析

Clinical Effect of Bipolar Electrocoagulation in the Treatment of Appendix Root and Mesangium during Laparoscopic Appendectomy

目的:评估腹腔镜阑尾切除术中应用双极电凝法处理阑尾根部及系膜的临床疗效。方法:选取2018年3月—2023年3月苏州市相城区第三人民医院普外科收治的急性阑尾炎患者70例作为研究对象,采用随机数字表法分为电凝组和对照组,各35例。两组均采用三孔腹腔镜阑尾切除术进行治疗,电凝组应用双极电凝法处理阑尾根部及系膜,对照组采用传统切割丝线结扎止血技术。比较两组术中出血量、术后疼痛程度和术后并发症发生情况。结果:电凝组术中出血量少于对照组,差异有统计学意义(P=0.02);术后,两组视觉模拟评分法评分均低于术前,电凝组低于对照组,差异有统计学意义(P<0.05);电凝组术后并发症总发生率低于对照组,差异有统计学意义(P=0.044)。结论:腹腔镜阑尾切除术中应用双极电凝法能够有效减少术中出血量,减轻术后疼痛,降低术后并发症发生率,可作为一种安全、有效的止血技术应用于临床。

Objective:To evaluate the clinical effect of bipolar electrocoagulation in the treatment of appendix root and mesangium during laparoscopic appendectomy.Methods:Seventy patients with acute appendicitis admitted to the department of general sur⁃gery of Suzhou Xiangcheng District Third People's Hospital from March 2018 to March 2023 were selected as the study objects,and were divided into electrocoagulation group and control group by random number table method,with thirty-five cases in each group.The two groups were treated by three-hole laparoscopic appendectomy.The electrocoagulation group was treated by bipolar electrocoagulation,while the control group was treated by traditional cutting thread ligation.The amount of blood loss,postopera⁃tive pain and postoperative complications were compared between the two groups.Results:The amount of blood loss in electroco⁃agulation group was less than that in control group,and the difference was statistically significant(P=0.02);After operation,the score of visual analogue score in both groups was lower than before operation,and that in electrocoagulation group was lower than control group,the difference was statistically significant(P<0.05);The total incidence of postoperative complications in electroco⁃agulation group was lower than that in control group,and the difference was statistically significant(P=0.044).Conclusion:The ap⁃plication of bipolar electrocoagulation in laparoscopic appendectomy can effectively reduce the amount of intraoperative blood loss,relieve postoperative pain and reduce the incidence of postoperative complications,and can be used as a safe and effective hemostatic technique in clinical practice.
