血尿酸、同型半胱氨酸联合检测在临床预测原发性高血压患者发生冠心病中的应用价值

Application Value of Serum Uric Acid and Homocysteine Combined Detection in Clinical Prediction of Coronary Heart Disease in Patients with Essential Hypertension

摘要 目的:探讨血尿酸(UA)、同型半胱氨酸(Hcy)联合检测对临床预测原发性高血压(PHTN)患者发生冠心病的应用价值。方法:选取2023年1—8月北京市昌平区天通苑南社区卫生服务中心接诊的PHTN合并冠心病患者48例作为观察组,另选取同期单纯PHTN患者48例作为对照组,以及同期健康体检者48例作为健康组。采集所有研究对象血液样本开展UA、Hcy检测,比较三组UA、Hcy差异;绘制受试者工作特征曲线(ROC曲线)分析血清UA、Hcy及联合检测预测PHTN患者发生冠心病的临床价值。结果:观察组血清UA、Hcy水平高于对照组和健康组,差异有统计学意义(P<0.001)。绘制ROC曲线显示,血清UA、Hcy及联合检测预测PHTN患者发生冠心病的曲线下面积值分别为0.816、0.847、0.894;联合检测预测价值最高。结论:UA、Hcy在预测PHTN患者冠心病发生中具有一定价值,联合检测可提高预测效能,便于临床及早察觉冠心病风险,做好针对性防治。 Objective:To investigate the value of serum uric acid(UA)and homocysteine(Hcy)combined detection in clinical pre⁃diction of coronary heart disease in patients with essential hypertension(PHTN).Methods:A total of forty-eight patients with PHTN complicated with coronary heart disease admitted to Beijing Changping District Tiantongyuan South Community Health Ser⁃vice Center from January to August 2023 were selected as the observation group,forty-eight patients with simple PHTN were se⁃lected as the control group,and forty-eight healthy subjects were selected as the healthy group during the same period.Blood samples of all subjects were collected for UA and Hcy detection,and the differences of UA and Hcy among the three groups were compared.Receiver operating characteristic curve(ROC curve)was drawn to analyze the clinical value of serum UA,Hcy and com⁃bined detection in predicting coronary heart disease in patients with PHTN.Results:The levels of serum UA and Hcy in observation group were higher than those in control group and healthy group,and the difference was statistically significant(P<0.001);ROC curve showed that the area under the curve of serum UA,Hcy and combined detection to predict coronary heart disease in patients with PHTN were 0.816,0.847 and 0.894,respectively;Combined detection had the highest predictive value.Conclusion:UA and Hcy have a certain value in predicting the occurrence of coronary heart disease in patients with PHTN.Combined detection can improve the prediction efficiency,facilitate the early detection of coronary heart disease risk,and do a good job in prevention and treatment.
