丹蛭降糖胶囊治疗早期糖尿病肾病的临床疗效观察

Clinical Observation of Danzhi Jiangtang Capsules in the Treatment of Early Diabetic Kidney Disease

目的观察丹蛭降糖胶囊治疗早期糖尿病肾病的临床疗效。方法选取80例早期糖尿病肾病患者,均来自于2021年6月至2022年12月安徽中医药大学第一附属医院内分泌科,随机分为对照组和观察组各40例,两组均给予常规治疗,观察组在对照组治疗基础上,同时给予丹蛭降糖胶囊口服,并随访12周,观察两组血糖指标、肾功能指标、中医证候积分、血清胱抑素C、超敏C反应蛋白、血清淀粉样蛋白A水平的变化及临床疗效。结果治疗后两组糖基化血红蛋白A1c、空腹血糖、餐后2 h血糖水平均较治疗前明显下降(P<0.05),且观察组下降程度更为明显(P<0.05);治疗后两组尿素氮、肌酐水平均较治疗前明显下降(P<0.05),肾小球滤过率水平均较治疗前上升(P<0.05),治疗后两组尿素氮、肌酐、肾小球滤过率水平比较差异无统计学意义(P>0.05);治疗后两组尿微量白蛋白、尿微量白蛋白/尿肌酐水平均较治疗前明显下降(P<0.05),且观察组尿微量白蛋白水平降低程度明显大于对照组(P<0.05);治疗后两组中医证候积分均较治疗前下降(P<0.05),且观察组降低更明显(P<0.05);观察组的总有效率明显优于对照组(P<0.05);治疗后两组血清胱抑素C、超敏C反应蛋白、血清淀粉样蛋白A水平均较治疗前明显下降(P<0.05),且观察组降低更明显(P<0.05)。结论丹蛭降糖胶囊治疗糖尿病肾病效果良好,同时能降低血清血清胱抑素C、超敏C反应蛋白和血清淀粉样蛋白A水平。

Objective To observe the clinical effect of Danzhi Jiangtang capsules with the effect of supplementing Qi,nourishing Yin and dredging collaterals,in the treatment of early diabetic kidney disease.Methods Eighty patients with early diabetic nephropathy were selected from the Department of Endocrinology,the First Affiliated Hospital of Anhui University of Traditional Chinese Medicine from June 2021 to December 2022,and randomly divided into control group and observation group,with 40 cases in each group.Both groups were given conventional treatment for diabetic kidney disease,and the observation group was given Danzhi Jiangtang capsulesorally at the same time on the basis of the treatment of the control group,and followed up for 12 weeks.The changes of blood glucose indexes,renal function indexes,Chinese medicine evidence points,serum cystatin C,ultrasensitive C-reactive protein,serum amyloid A levels and clinical efficacy in both groups were observed.Results Glycosylated haemoglobin A1c,fasting blood glucose and 2-hour postprandial blood glucose levels decreased significantly in both groups after treatment compared with those before treatment(P<0.05),and the degree of decrease was more obvious in the observation group(P<0.05).
