某院2021—2022年门诊和急诊处方点评情况分析

Analysis of Outpatient and Emergency Prescription Comment in a Certain Hospital from 2021 to 2022

目的 对南通市通州区石港中心卫生院近2年来的门诊和急诊处方进行点评,分析处方不合理发生情况,并提出对策。方法 采用分层随机抽样方法抽取2021年1月—2022年12月南通市通州区石港中心卫生院门诊和急诊处方158 577张为点评对象,分析处方不合理发生情况。结果 158 577张处方中不合理处方3 954张,不合理率为2.49%;其中急诊处方不合理率为2.50%,门诊处方不合理率为2.49%。2021年处方不合理率为2.50%,急诊和门诊处方不合理率均为2.50%。2022年处方不合理率为2.49%,门诊和急诊处方不合理率均为2.49%。3 954张不合理处方中以用药不适宜处方为主,占62.77%,不规范处方占37.23%。结论 该卫生院存在较高的处方不合理率,其中处方不合理又以用药不适宜为主,应加强处方相关的监管力度,以及临床医师合理书写处方等相关的培训,以提高处方合理率和用药合理性。

Objective Evaluate the outpatient and emergency prescriptions of Shigang Health Center in Tongzhou District,Nantong City in the past two years,analyze the occurrence of unreasonable prescriptions,and propose countermeasures.Methods A stratified random sampling method was used to select 158,577 outpatient and emergency prescriptions from Shigang Health Center in Tongzhou District,Nantong City from January 2021 to December 2022 as the evaluation objects,and the occurrence of unreasonable prescriptions was analyzed.Results Out of 158,577 prescriptions,3,954 were found to be unreasonable,with an unreasonable rate of 2.49%.The unreasonable rate of emergency prescriptions is 2.50%,and the unreasonable rate of outpatient prescriptions is 2.49%.The unreasonable prescription rate in 2021 was 2.50%,and the unreasonable prescription rates in both emergency and outpatient departments were 2.50%.The unreasonable prescription rate in 2022 was 2.49%,and the unreasonable prescription rates for outpatient and emergency departments were both 2.49%.Out of the 3,954unreasonable prescriptions,62.77% were found to have inappropriate medication,while 37.23% were non-standard prescriptions.Conclusion There is a high rate of prescription irrationality in this health center,mainly due to inappropriate medication.Therefore,it is necessary to strengthen the supervision of prescription related measures and provide training for clinical physicians on reasonable prescription writing to improve prescription rationality and medication rationality.
