PET-CT对非小细胞肺癌淋巴结转移的诊断效果

Diagnostic Effect of PET-CT on Lymph Node Metastasis in Non-small Cell Lung Cancer

摘要 目的 分析正电子发射计算机断层显像(position emission tomography,PET)-计算机断层扫描(computed tomography,CT)对非小细胞肺癌(non-small cell lung cancer,NSCLC)淋巴结转移的诊断效果。方法回顾性分析2021年4月—2023年9月天津市宝坻区人民医院收治的110例NSCLC患者的临床资料,所有患者均接受多层螺旋CT检查和PET-CT检查。以病理结果为金标准,比较CT和PET-CT检查对NSCLC淋巴结转移的诊断效能。比较NSCLC淋巴结转移患者与未转移患者CT检查时病灶的碘浓度和水浓度,以及PET-CT检查的最大标准摄取值和淋巴结短径。结果 病理检查结果显示,110例NSCLC患者中淋巴结转移39例、未转移71例;多层螺旋CT检查显示淋巴结转移22例、未转移47例;PET-CT检查显示淋巴结转移33例、未转移63例。PET-CT检查诊断淋巴结转移的敏感性、特异性、准确性、阳性预测值、阴性预测值均高于多层螺旋CT检查,差异有统计学意义(P<0.05)。PET-CT检查显示:淋巴结转移患者最大标准摄取值和淋巴结短径高于淋巴结未转移患者,差异有统计学意义(P<0.05)。多层螺旋CT检查显示:淋巴结转移患者碘浓度低于淋巴结未转移患者,差异有统计学意义(P<0.05);淋巴结转移与未转移患者水浓度差异无统计学意义(P>0.05)。结论 PET-CT检查有助于判断NSCLC患者淋巴结是否发生转移,且该检查方式的诊断效能优于多层螺旋CT,值得推广使用。 Objective Analyze the diagnostic effect of positron emission tomography(PET)-computed tomography(CT) on lymph node metastasis in non-small cell lung cancer(NSCLC).Methods Retrospective analysis of clinical data was conducted in 110 NSCLC patients admitted to Tianjin Baodi District People's Hospital from April 2021 to September 2023.All patients underwent multi-slice spiral CT and PET-CT examinations.Using pathological results as the gold standard,compare the diagnostic efficacy of CT and PET-CT for lymph node metastasis in NSCLC.Compare the iodine concentration and water concentration of the lesion during CT examination between NSCLC lymph node metastasis patients and non-metastasis patients,as well as the maximum standard uptake value and lymph node diameter during PET-CT examination.Results The pathological examination results showed that among 110 NSCLC patients,39 cases had lymph node metastasis and 71 cases had non-metastasis.Multislice spiral CT examination showed 22 cases of lymph node metastasis and 47 cases of non-metastasis.PET-CT examination showed 33 cases of lymph node metastasis and 63 cases of non-metastasis.The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of PET-CT in diagnosing lymph node metastasis were significantly higher than those of multi-slice spiral CT(P<0.05).PET-CT examination showed that the maximum standard uptake value and lymph node short diameter in patients with lymph node metastasis were significantly higher than those in patients without lymph node metastasis(P<0.05).Multi-slice spiral CT examination showed that the iodine concentration in patients with lymph node metastasis was significantly lower than that in patients without lymph node metastasis(P <0.05).There was no significant difference in water concentration between patients with lymph node metastasis and those without metastasis(P>0.05).Conclusion PET-CT examination helps to determine whether lymph node metastasis has occurred in NSCLC patients,and the diagnostic efficiency of this examination method is better than that of multi-slice spiral CT,which is worth promoting and using.
