血清CC16在IPF和大气细颗粒物相关ILD之间的表达差异及分析

Expression difference and analysis of serum CC16 in IPF and ILD associated with atmospheric fine particulate matter

摘要 目的探讨特发性肺间质纤维化(idiopathic pulmonary fibrosis,IPF)和大气细颗粒物长期职业暴露相关的间质性肺疾病(interstitial lung disease,ILD)中血清人克拉拉细胞分泌蛋白(clara cell secretory protein,CC16)的表达差异及二者肺泡—毛细血管屏障损伤过程的异同。方法收集2019年11月—2020年5月于哈尔滨医科大学附属第一医院呼吸与危重症医学科确诊的IPF患者20例作为A组,大气细颗粒物长期职业暴露相关的ILD患者18例作为B组,健康体检志愿者18人作为C组。收集3组研究对象的临床资料并使用ELISA检测血清CC16浓度,采用单因素方差分析3组间血清CC16水平的差异,其中每2组之间血清CC16水平的差异性分析采用Tukey HSD检验。结果A组血清CC16浓度[(1318.15±96.93)pg/ml]高于C组[(1233.53±110.82)pg/ml],B组血清CC16浓度[(1029.56±86.38)pg/ml]低于C组。单因素方差分析显示,血清CC16水平在A组、B组和C组之间的差异有统计学意义(P<0.05);进一步使用Tukey HSD检验显示,A组与C组、B组与C组、A组与B组之间血清CC16水平差异也有统计学意义(P<0.05)。结论IPF和大气细颗粒物长期职业暴露相关的ILD血清CC16浓度存在显著差异。 Objective To investigate the expression difference of serum clara cell secretory protein(CC16)between idiopathic pulmonary fibrosis(IPF)and the interstitial lung disease(ILD)associated with long-term occupational exposure to atmospheric fine particulate matter,and to analyze the similarities and differences of alveolar-capillary barrier injury process.Methods The IPF patients diagnosed in the department of respiratory and critical care medicine of the First Affiliated Hospital of Harbin Medical University from November 2019 to May 2020 were collected as group A(n=20),ILD patients with long-term occupational exposure to atmospheric fine particulate matter as group B(n=18)and healthy volunteers as group C(n=18).The clinical data of the three groups were collected and the concentration of serum CC16 was detected by ELISA.One-factor analysis of variance was used to analyze the difference of serum CC16 levels among the three groups and the Tukey HSD test was used to analyze the difference of serum CC16 levels between each two groups.Results The concentration of serum CC16 in group A[(1318.15±96.93)pg/ml]was higher than that in group C[(1233.53±110.82)pg/ml],and the concentration of serum CC16 in group B[(1029.56±86.38)pg/ml]was lower than that in group C.The results of one-factor analysis of variance showed that the serum CC16 level was significantly different among group A,group B and group C(P<0.05).The Tukey HSD test results showed that there were significant differences in serum CC16 levels between group A and group C,group B and group C,and group A and group B(P<0.05).Conclusion There was significant difference in the concentration of serum CC16 between IPF and the ILD associated with long-term occupational exposure to atmospheric fine particulate matter.
