关节粘连传统松解术联合中药熏洗对运动性膝关节损伤的治疗效果

Effect of traditional joint adhesion lysis combined with herbal fumigation on exercise-induced knee osteoarthritis

目的观察关节粘连传统松解术联合中药熏洗对运动性膝关节损伤的治疗效果。方法将60例运动性膝关节损伤患者按照随机数字表法分为2组,对照组30例予西药治疗,治疗组30例在对照组治疗基础上予关节粘连传统松解术联合中药熏洗治疗。疗程4周。比较2组治疗前后Lysholm膝关节评分、180°/s、60°/s等速运动的屈膝肌峰力矩、伸膝肌峰力矩、疼痛视觉模拟评分(VAS)、简明健康状况量表(SF-36)评分,并统计2组疗效。结果治疗组总有效率86.67%(26/30),对照组总有效率66.67%(20/30),治疗组临床疗效优于对照组(P<0.05)。2组治疗后Lysholm膝关节评分均较本组治疗前升高(P<0.05),且治疗组治疗后高于对照组(P<0.05)。2组治疗后等速运动伸肌峰力矩、等速运动屈肌峰力矩均较本组治疗前升高(P<0.05),且治疗组治疗后均高于对照组(P<0.05)。2组治疗后疼痛VAS均较本组治疗前降低(P<0.05),且治疗组治疗后低于对照组(P<0.05)。2组治疗后SF-36各项评分均较本组治疗前升高(P<0.05),且治疗组治疗后高于对照组(P<0.05)。结论关节粘连传统松解术联合中药熏洗对运动性膝关节损伤的治疗效果较好,值得临床推广。

Objective To study the effect of traditional joint adhesion lysis combined with herbal fumigation on exercise-induced knee osteoarthritis(KOA).Method Sixty patients with exercise-induced KOA were randomized 1:1 into the control group and the treatment group.All patients received Western medicine treatment,patients in the treatment group additionally received traditional joint adhesion lysis combined with herbal fumigation.A 4-week of treatment was performed,aiming to compare the Lysholm score,isokinetic peak torque of knee flexors/extensor(60 deg/s and 180 deg/s),visual analogue scale(VAS)for pain,and the 36 item Short-form Health Survey(SF-36)scores before and after treatment,and the curative effect of the two groups was assessed.Result The total effective rate of the treatment group was better than that of the control group(86.67%[26/30]vs 66.67%[20/30],P<0.05).Patients in the both groups presented significantly increased Lysholm score(P<0.05),improved isokinetic peak torque of knee flexors/extensor(P<0.05)and elevated SF-36 scores(P<0.05),and significantly decreased VAS scores(P<0.05)than those before treatment,and the improvements were more pronounced in the treatment group(P<0.05).Conclusion The effect of joint adhesion lysis combined with herbal fumigation on exercise-induced KOA is ideal and worthy of clinical promotion.
