老年慢性心力衰竭临床特点及药物治疗分析

Clinical Characteristics and Drug Treatment Analysis of Elderly Chronic Heart Failure

目的探讨老年慢性心力衰竭临床特点及药物使用特点。方法选取医院2016年12月1日至2019年12月1日收治的65岁以上的慢性心力衰竭患者70例的剂量,按是否进行药物调整分为对照组和试验组,各35例。对照组患者予传统基石性药物治疗,试验组患者在对照组治疗基础上调整贝那普利、地高辛、螺内酯、呋塞米和比索洛尔的剂量,分析老年慢性心力衰竭临床特点。结果两组患者的舒张压、肾功能、血红蛋白差异无统计学意义(P>0.05),收缩压、心率、肝功能指标差异显著(P<0.05)。试验组患者左心室射血分数、胸部X线摄片与心电图好转、B型尿钠肽≥100 ng/L、6 min步行试验步行距离>450 m均显著优于对照组(P<0.05)。试验组使用比索洛尔的患者左心室射血分数和心率-血压双乘积均优于使用美托洛尔的患者(P<0.05)。结论临床治疗老年慢性心力衰竭,要依据患者疾病特点给予个体化治疗,并结合患者临床表现选择适宜药物。

Objective To investigate the clinical characteristics and drug use characteristics of elderly patients with chronic heart failure.Methods A total of 70 patients with chronic heart failure over 65 years old admitted to the hospital from December 1,2016 to December 1,2019 were divided into a control group and an experimental group based on whether medication adjustment was performed,with 35 cases in each group.The control group was treated with traditional cornerstone drugs,while the experimental group regulated the dose of benazepril,digoxin,spironolactone,furosemide,and bisoprolol on the basis of the control group treatment.The clinical characteristics of elderly patients with chronic heart failure were analyzed.Results There was no significant difference in diastolic blood pressure,renal function,and hemoglobin between the two groups(P>0.05),and there were significant differences in systolic blood pressure,heart rate,and liver function indicators(P<0.05);The patients in the experimental group who used bisoprolol had better left ventricular ejection fraction and heart rate blood pressure double product than those who used metoprolol(P<0.05).Conclusion In clinical treatment of elderly chronic heart failure,individualized treatment should be given based on the patient's disease characteristics,and appropriate drugs should be selected according to the patient's clinical manifestations.
