机械通气治疗ICU多发伤致急性呼吸窘迫综合征临床观察

Clinical Observation of Mechanical Ventilation in the Treatment of Acute Respiratory Distress Syndrome Caused by Multiple Injuries in the ICU

摘要 目的探讨机械通气治疗ICU多发伤致急性呼吸窘迫综合征的临床效果。方法选取医院2022年1月至12月收治的ICU多发伤致急性呼吸窘迫综合征患者40例,按双盲法分为对照组和观察组,各20例。两组患者均予常规治疗,观察组患者加用机械通气。结果观察组临床有效率为90.00%,显著高于对照组的60.00%(P<0.05)。观察组患者治疗后的收缩压、动脉血氧分压、动脉血二氧化碳分压、氧合指数均显著高于对照组,舒张压和心率均显著低于对照组(P<0.05);第1秒用力呼气容积及呼气峰值流速均显著高于对照组(P<0.05);机械通气时间及ICU停留时间均显著短于对照组(P<0.05)。结论机械通气用于ICU多发伤致急性呼吸窘迫综合征的治疗,可有效改善患者的血气指标及肺功能指标。 Objective To investigate the clinical efficacy of mechanical ventilation in the treatment of acute respiratory distress syndrome caused by multiple injuries in the ICU.Methods Forty ICU patients with acute respiratory distress syndrome caused by multiple injuries admitted to the hospital from January to December 2022 were selected and divided into the control group and the observation group by the double-blind method,with 20 cases in each group.Both groups received routine treatment,on this basis,the observation group received mechanical ventilation.Results The clinical effective rate in the observation group was 90.00%,which was significantly higher than 60.00%in the control group(P<0.05).The systolic blood pressure,arterial oxygen partial pressure,arterial carbon dioxide partial pressure,and oxygenation index in the observation group after treatment were significantly higher than those in the control group,while the diastolic blood pressure and heart rate in the observation group were significantly lower than those in the control group(P<0.05).The forced expiratory volume and peak expiratory flow rate in the first second in the observation group were significantly higher than those in the control group(P<0.05).The mechanical ventilation time and ICU stay time in the observation group was significantly shorter than that in the control group(P<0.05).Conclusion Mechanical ventilation used for the treatment of acute respiratory distress syndrome caused by multiple injuries in the ICU can effectively improve the blood gas and lung function indicators of patients.
