儿童慢性分泌性中耳炎行腺样体等离子消融术的疗效

Effect of Adenoid Plasma Ablation on Chronic Secretory Otitis Media in Children

目的探讨儿童分泌性中耳炎行腺样体等离子消融术治疗的临床效果。方法选取2020年1月—2023年7月南京市六合区人民医院收治的慢性分泌性中耳炎患儿60例为研究对象,以随机数表法分为对照组(n=30,鼻内镜下腺样体切除)与观察组(n=30,低温等离子腺样体消融术治疗)。比较两组手术及术后恢复指标、炎症因子水平和并发症发生率。结果观察组手术时间短于对照组,出血量低于对照组,耳鸣消失、听力恢复、中耳积液时间均短于对照组,差异有统计学意义(P均<0.05)。术后,观察组肿瘤坏死因子-α、白介素-6、白介素-2均低于对照组,差异有统计学意义(P均<0.05)。观察组并发症总发生率(3.33%)低于对照组(26.67%),差异有统计学意义(χ^(2)=4.706,P<0.05)。结论对儿童分泌性中耳炎采用腺样体等离子消融术治疗效果确切,操作时间短且患儿损伤小,有利于患儿术后恢复,炎症反应轻微,并发症风险低。

Objective To investigate the clinical effect of adenoid plasma ablation for secretory otitis media in children.Methods 60 cases of children with chronic secretory otitis media admitted to Liuha District People′s Hospital of Nanjing City from January 2020 to July 2023 were selected as study subjects,and were divided into the control group(n=30,naso-endoscopic adenoidectomy)and the observation group(n=30,low-temperature plasma adenoid ablation treatment)by random number table method.The surgical and postoperative recovery indexes,inflammatory factor levels and complication rates were compared between the two groups.Results The operation time of the observation group was shorter than that of the control group,the amount of bleeding was lower than that of the control group,and the time of tinnitus disappearance,hearing recovery,and middle ear effusion were shorter than those of the control group,and the differences were statistically significant(all P<0.05).After surgery,tumor necrosis factor-α,interleukin-6 and interleukin-2 were lower in the observation group than those in the control group,and the differences were statistically significant(all P<0.05).The total complication rate of the observation group(3.33%)was lower than that of the control group(26.67%),and the difference was statistically significant(χ^(2)=4.706,P<0.05).Conclusion Adenoid plasma ablation for secretory otitis media in children has definite effect,short operation time and little injury,which is conducive to postoperative recovery of patients,mild inflammatory reaction and low risk of complications.
