牙周翻瓣术联合根管治疗对牙髓牙周综合征患者牙周健康指标及炎症因子水平的影响

The effect of periodontal flap surgery combined with root canal therapy on periodontal health indicators and inflammatory factor levels in patients with pulp and periodontal syndrome

目的探讨应用牙周翻瓣术联合根管治疗术治疗牙髓牙周综合征的疗效及对牙周健康指标、炎症因子水平影响.方法选取2021年2月-2023年4月收治的牙髓牙周综合征患者80例,按随机数字表法原则分成观察组(n=40)、对照组(n=40).对照组患者应用常规根管治疗,观察组患者应用牙周翻瓣术联合根管治疗.对比两组患者的治疗效果、治疗前后牙周健康指标[菌斑指数(PLI)、牙周袋探诊深度(PD)、牙龈指数(GI)、龈沟出血指数(SBI)]及治疗前后炎症因子[C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]水平.结果观察组的治疗总有效率(90.00%)高于对照组(70.00%),差异有统计学意义(P<0.05);两组患者治疗3个月后的PLI、PD、GI、SBI、CRP、IL-6、TNF-α水平均低于治疗前,且观察组治疗3个月后的各指标水平低于对照组,差异有统计学意义(P<0.05).结论对于牙髓牙周综合征,应用牙周翻瓣术联合根管治疗术治疗效果满意,可改善患者牙周健康状况,降低炎症因子水平,值得推广.

Objective To explore the efficacy of periodontal flap surgery combined with root canal therapy in the treatment of pulpal periodontal syndrome and its impact on periodontal health knowledge and inflammatory factor levels.Method 80 patients with pulpal periodontal syndrome admitted from February 2021 to April 2023 were randomly divided into an observation group(n=40)and a control group(n=40)according to the principle of a random number table.The control group patients received conventional root canal therapy,while the observation group patients received periodontal flap surgery combined with root canal therapy.Compare the treatment effectiveness of two groups of patients,compare the periodontal health index(PLI),periodontal pocket probing depth(PD),gingival index(GI),and sulcus bleeding index(SBI)before and after treatment,and compare the inflammatory factors[C-reactive protein(CRP),interleukin-6(IL-6),and tumor necrosis factor-α(TNF-α)]horizontal before and after treatment.Result The total effective rate of treatment in the observation group(90.00%)was higher than that in the control group(70.00%),with a statistically significant difference(P<0.05);two groups of patients were treated for 3 months in PLI,PD,GI,SBI,CRP,IL-6,TNF-αthe levels were all lower than before treatment,and the levels of various indicators in the observation group were lower than those in the control group after 3 months of treatment,with statistical significance(P<0.05).Conclusion For pulpal periodontal syndrome,the application of periodontal flap surgery combined with root canal therapy has a satisfactory therapeutic effect,can improve the patient's periodontal health status,reduce the level of inflammatory factors,and is worth promoting.
