生化检查联合肌电图对诊断糖尿病周围神经病变的临床应用价值

摘要 目的 探讨糖尿病周围神经病变诊断中生化检查联合肌电图的价值。方法 回顾性选取2020年2月—2022年2月泉州市第一医院收治的90例糖尿病患者的临床资料,依据有无周围神经病变分为有周围神经病变组(n=40)、无周围神经病变组(n=50)。比较两组患者血压及生化指标、肌电图数据。结果 有周围神经病变组的胱抑素C(Cystatin C, Cys-C)、尿酸、肌酐水平及肾小球滤过率均高于无周围神经病变组,差异有统计学意义(P均<0.05)。有周围神经病变组的腓神经感觉神经左右小腿波幅、右小腿传导速度均低于无周围神经病变组,运动神经左踝潜伏期长于无周围神经病变组,左踝波幅、左腓小头传导速度均低于无周围神经病变组,左右腓小头潜伏期均长于无周围神经病变组,正中神经的感觉神经左右腕潜伏期均长于无周围神经病变组,左右腕波幅、左右腕传导速度均低于无周围神经病变组,正中神经的运动神经左右腕潜伏期、左右肘潜伏期均长于无周围神经病变组,左肘波幅、左右腕传导速度、右肘传导速度均低于无周围神经病变组,差异有统计学意义(P均<0.05)。结论 生化检查联合肌电图对糖尿病周围神经病变的临床应用价值高。 Objective To investigate the value of biochemical examination combined with electromyography in the di-agnosis of diabetic peripheral neuropathy.Methods The clinical data of 90 diabetic patients admitted to the Quan-zhou First Hospital from February 2020 to February 2022 were retrospectively selected,and the were divided into a group with peripheral neuropathy(n=40)and a group without peripheral neuropathy(n=50)according to whether they had peripheral neuropathy.Blood pressure,biochemical indexes and electromyography data were compared between the two groups of patients.Results The levels of cystatin C(Cys-C),uric acid,creatinine and glomerular filtration rate in the group with peripheral neuropathy were higher than those in the group without peripheral neuropathy,and the differences were statistically significant(all P<0.05).The left and right leg amplitude and right leg conduction veloc-ity of the peroneal sensory nerve in the group with peripheral neuropathy were lower than those in the group without peripheral neuropathy,and the latency of the left ankle of the motor nerve was longer than that in the group without pe-ripheral neuropathy,and the left ankle amplitude and left fibular head conduction velocity were lower than those in the group without peripheral neuropathy,and the latency of the left and right fibular head was longer than that in the group without peripheral neuropathy.The latency of left and right wrist of sensory nerve of median nerve were longer than that of group without peripheral neuropathy,and the latency of left and right wrist and the conduction velocity of left and right wrist were lower than those of group without peripheral neuropathy,and the latency of left and right wrist of motor nerve of median nerve were longer than those of group without peripheral neuropathy,and the fluctuation of left elbow,the conduction velocity of right elbow were lower than that of group without peripheral neuropathy,and the differences were statistically significant(all P<0.05).Conclusion Biochemical examination combined with electromy-ography has high clinical value in the treatment of diabetic peripheral neuropathy.
