彩色多普勒超声心动图诊断心脏瓣膜病的临床价值

Clinical value of color Doppler echocardiogram in the diagnosis of valvular heart disease

目的探讨彩色多普勒超声心动图诊断心脏瓣膜病的临床价值。方法回顾性分析2019年2月至2021年10月于萍乡市人民医院就诊的82例疑似心脏瓣膜病患者的临床资料,患者均接受彩色多普勒超声心动图诊断,以术后病理检查结果为金标准,分析彩色多普勒超声心动图对心脏瓣膜病的诊断价值。结果82例疑似心脏瓣膜病患者,术后病理检查确诊79例,彩色多普勒超声心动图检出76例;彩色多普勒超声心动图诊断灵敏度为94.94%、特异度为66.67%、准确率为93.90%。彩色多普勒超声心动图与术后病理检查瓣膜病变分期情况比较差异无统计学意义。彩色多普勒超声心动图检查检出主动脉瓣瓣膜病变30例,二尖瓣瓣膜病变42例,其他4例;术后病理检查检出主动脉瓣瓣膜病变32例,二尖瓣瓣膜病变43例,其他4例;彩色多普勒超声心动图与术后病理检查对主动脉瓣和二尖瓣的混合型瓣叶、瓣叶钙化、瓣叶增厚且粘连检出率比较差异无统计学意义。结论彩色多普勒超声心动图对心脏瓣膜病诊断具有较高的灵敏度和准确率,能对病情发展的严重程度进行有效分期,并探查瓣膜病变的类型、病变位置,对疾病诊断及疗效判定具有较高应用价值。

Objective To explore the clinical value of color Doppler echocardiogram in the diagnosis of valvular heart disease.Methods Clini-cal data of 82 patients with suspected valvular heart disease treated in Pingxiang People's Hospital from February 2019 to October 2021 were retro-spectively analyzed,all patients were diagnosed by color Doppler echocardiography,the value of color Doppler ultrasound in the diagnosis of valvu-lar heart disease was analyzed with the results of postoperative pathological examination as the gold standard.Results Among 82 patients with sus-pected valvular heart disease,79 cases were confirmed by postoperative pathological examination,and 76 cases were detected by color Doppler echocardiograph;the diagnostic sensitivity,specificity and accuracy of color Doppler echocardiography were 94.94%,66.67%and 93.90%.There were no significant differences in the staging of valvular lesions between color Doppler echocardiography and postoperative pathological examina-tion.Color Doppler echocardiography detected 30 cases of aortic valve disease,42 cases of mitral valve disease,and 4 cases of other diseases;post-operative pathological examination detected 32 cases of aortic valve disease,43 cases of mitral valve disease,4 cases of other diseases;there was no significant difference in the detection rate of mixed aortic and mitral leaflets,calcification,thickening and adhesion between color Doppler echocar-diography and postoperative pathological examination.Conclusion Color Doppler echocardiogram has high diagnostic sensitivity and accuracy in valvular heart disease,which can effectively stage the severity of disease development,and probe the types and sites of valvular lesions,with high ap-plication value in disease diagnosis and the determination of curative effect.
