中医针灸科与骨伤科住院医师规范化培训中实施中医迷你临床演练评估教学法对住院医师综合素质及实践能力的影响

Influences of Traditional Chinese Medicine mini-clinical evaluation exercise teaching method in the resident standardized training on comprehensive quality and practical ability of Traditional Chinese Medicine acupuncture and orthopaedic resident physicians

目的探究中医迷你临床演练评估(TCM-Mini-CEX)教学法应用于中医针灸科、骨伤科住院医师规范化培训中的效果。方法选取2020年5月至2021年10月于梧州市中医医院进行规范化培训的60名中医针灸科、骨伤科住院医师作为研究对象,采用电脑随机数字法分为对照组与研究组,每组30名。对照组实施常规培训,研究组在对照组基础上联合TCM-Mini-CEX教学法。比较两组综合素质、实践能力及对培训模式的满意度。结果培训后,两组职业道德、医疗法规、医患沟通、临床思维、基本知识、基本理论、基本技能、科研能力评分均高于培训前,且研究组高于对照组,差异有统计学意义(P<0.05)。培训后,两组问诊能力、体格检查、辅助检查、技能操作评分均高于培训前,且研究组高于对照组,差异有统计学意义(P<0.05)。研究组培训总满意率高于对照组,差异有统计学意义(P<0.05)。结论TCM-Mini-CEX教学法应用于中医针灸科、骨伤科住院医师的规范化培训效果显著,可明显提高住院医师综合素质能力及实践能力,且对培训模式较为满意。

Objective To explore the clinical effect of Traditional Chinese Medicine mini-clinical evaluation exercise(TCM-Mini-CEX)teach-ing method applied to the resident standardized training of TCM acupuncture and orthopaedic resident physicians.Methods 60 resident physicians in the department of TCM Acupuncture and Orthopedics who received standardized training in Wuzhou Traditional Chinese Medicine Hospital from May 2020 to October 2021 were collected as the research subjects,and they were separated into two groups according computer random number method,with 30 cases in each group.The control group was given routine training,and the study group was combined with TCM-Mini-CEX teach-ing method on the basis of the control group.The comprehensive quality,practical ability and satisfaction with the training mode were compared be-tween the two groups.Results After training,the scores of professional ethics,medical regulations,doctor-patient communication,clinical thinking,basic knowledge,basic theory,basic skills and scientific research ability of the two groups were higher than those before training,and the study group was higher than the control group,the differences were statistically significant(P<0.05).After training,the scores of inquiry ability,physical examination,auxiliary examination and skill operation of the two groups were higher than those before training,and the study group were higher than the control group,the differences were statistically significant(P<0.05).The total satisfaction rate of training in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The application of TCM-Mini-CEX teaching method in the resident standardized training of residents in acupuncture and moxibustion department of TCM and orthopedics and traumatology de-partment has significant effect,which can obviously improve the comprehensive quality and practical ability of residents,and is satisfied with the training mode.
