改良透明压模保持器联合直丝弓矫治器对安氏Ⅱ类错[牙合]畸形患儿的临床疗效

Clinical effect of modified transparent pressure mold retainer combined with straight wire appliance on children with ankylosing classⅡmalocclusion

目的探讨改良透明压模保持器联合直丝弓矫治器对安氏Ⅱ类错[牙合]畸形患儿的临床疗效。方法选取2016年11月至2018年11月于北京大学首钢医院门诊治疗的58例安氏Ⅱ类错[牙合]畸形患儿作为研究对象,按照随机数字表法分为对照组与观察组,各29例。对照组采用直丝弓矫治器治疗,观察组采用改良透明压模保持器联合直丝弓矫治器治疗,比较两组治疗前后头颅侧位X线测定数据(上齿槽、下齿槽、上下齿槽座角和上颌与下颌、上颌中切牙与上颌、下颌中切牙与下颌平面夹角及上切牙切缘点、上切牙根尖点与FHp平面距离)。结果治疗1年后,两组上颌平面与下颌、上颌中切牙与上颌、下颌中切牙与下颌平面夹角及上切牙切缘点、上切牙根尖点与FHp平面距离均优于治疗前,且观察组上颌中切牙与上颌、下颌中切牙与下颌平面夹角及上切牙切缘点、上切牙根尖点与FHp平面距离均优于对照组,差异有统计学意义(P<0.05);两组其余指标两两比较差异无统计学意义。结论改良透明压模保持器联合直丝弓矫治器治疗安氏Ⅱ类错[牙合]畸形矫治效果显著,可减少后牙支抗的消耗,提高患儿的舒适度和美观度,值得在口腔正畸学领域推广和应用。

Objective To investigate the clinical effect of modified transparent pressure mold retainer combined with straight wire appliance on children with ankylosing classⅡmalocclusion.Methods 58 patients with ankylosing classⅡmalocclusion who were treated in the outpatient department of Peking University Shougang Hospital from November 2016 to November 2018 were selected as the research subjects,they were divided into the control group and the observation group according to the random number table method,with 29 cases in each group.The control group was treated with straight wire appliance,and the observation group was treated with modified transparent pressure mold retainer combined with straight wire appliance,the lateral cephalometric X-ray data(upper alveolar,lower alveolar,upper and lower alveolar seat angle and maxillary and mandibular,maxillary central incisor and maxillary,mandibular central incisor and mandibular plane angle and upper incisor incisal margin,upper incisor root apex and FHp plane distance)were compared between the two groups before and after treatment.Results After 1 year of treatment,the angle between maxillary plane and mandible,maxillary central incisor and maxillary,mandibular central incisor and mandibular plane,the distance between upper incisor incisal edge point,upper incisor root apex point and FHp plane in the two groups were better than those before treatment,and the angle between maxillary central incisor and maxillary,mandibular central incisor and mandibular plane,the distance between upper incisor incisal edge point,upper incisor root apex point and FHp plane in the observation group were better than those in the control group,the differences were statistically significant(P<0.05),but there was no significant difference in the other indexes between the two groups.Conclusion The modified transparent pressure mold retainer combined with straight wire appliance has significant effect in the treatment of ankylosing classⅡmalocclusion,reduces the consumption of posterior teeth anchorage,and improves the comfort and aesthetics of children,it is worthy of promotion and application in the field of orthodontics.
