前列腺素E2与糖尿病视网膜病变患者炎症状态及血管内皮生长因子的关系

Relationship between prostaglandin E2 level and inflammatory status and vascular endothelial growth factor in patients with diabetic retinopathy

目的探讨前列腺素E2(PGE2)水平与糖尿病视网膜病变(DR)患者炎症状态以及血管内皮生长因子(VEGF)的关系。方法选取2021年1月至2023年5月接受治疗的DR患者80例作为DR组,将其分为非增生性糖尿病视网膜病变(NPDR)组和增生性糖尿病性视网膜病变(PDR)组。另选取同期治疗的单纯2型糖尿病患者50例作为糖尿病组。检测各组的血清PGE2、白介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)、VEGF的水平。结果DR组的糖尿病病程长于糖尿病组,空腹血糖、糖化血红蛋白(HbA1c)、IL-1β、TNF-α、VEGF、PGE2水平均高于糖尿病组(P<0.05);PDR组的糖尿病病程长于NPDR组,空腹血糖、HbA1c、IL-1β、TNF-α、VEGF、PGE2水平均高于NPDR组(P<0.05);Pearson分析显示,DR患者血清PGE2水平与IL-1β、TNF-α、VEGF水平均呈正相关(P<0.05);Logistic多元回归显示,血清IL-1β、TNF-α、VEGF、PGE2水平过高以及糖尿病病程过长均是2型糖尿病患者发生DR的危险因素(P<0.05)。血清PGE2诊断DR的曲线下面积为0.808(95%CI:0.735~0.881)。结论PGE2在DR患者血清中呈异常高表达,对DR有一定的辅助诊断价值,其表达水平与患者的炎症状态和VEGF密切相关。

Objective To investigate the relationship between the level of prostaglandin E2(PGE2)and the inflammatory status and vascular endothelial growth factor(VEGF)in patients with diabetic retinopathy(DR).Methods From January 2021 to May 2023,totally 80 DR patients and 50 simple type 2 diabetes mellitus(T2DM)patients who were treated in our hospital were selected as the DR group and diabetes group,respectively.The patients in the DR group were divided into non-proliferative diabetic retinopathy(NPDR)group and proliferative diabetic retinopathy(PDR)group.The serum PGE2,interleukin 1β(IL-1β),tumor necrosis factorα(TNF-α)and VEGF in groups were included as comparators.Results The duration of diabetes in DR group was significantly longer than that in diabetes group,and which was significantly longer in PDR group than NPDR group(P<0.05).The levels of fasting blood glucose(FBG),hemoglobin A1c(HbA1c),IL-1β,TNF-α,VEGF and PGE2 in DR group were significantly increased than those in diabetes group(P<0.05),and which increased notably in PDR group compared with NPDR group(P<0.05).Pearson analysis showed a positive correlation between serum PGE2 levels and IL-1β,TNF-α,VEGF in DR patients(P<0.05).Logistic multiple regression showed that significantly high serum IL-1β,TNF-α,VEGF,PGE2,and significantly long duration of diabetes were risk factors for DR in T2DM patients(P<0.05).The area under the curve(AUC)for diagnosing DR with serum PGE2 was 0.808(95%CI:0.735-0.881).Conclusion For DR patients,serum PGE2 shows abnormally high expression,which can play a certain auxiliary diagnostic value.Its expression level is closely related to inflammatory status and VEGF.
