地佐辛对老年髋关节置换术后认知功能、负性情绪的影响

Effect of Diazoxide on Cognitive Function and Negative Emotions After Hip Arthroplasty in the Elderly

目的探讨地佐辛对老年髋关节置换术后认知功能、负性情绪的影响。方法将本院2020年2月~2023年3月共92例髋关节置换术患者按抛硬币随机法分为对照组与观察组,分别为45例,47例。对照组术中采用瑞芬太尼、丙泊酚维持麻醉,观察组在对照组基础上联合运用地佐辛。比较两组患者术前、术后6h、24h、3d、7d蒙特利尔认知评估(Montreal cognitive assessment,MoCA)、医院焦虑抑郁量表(Hospital Anxiety and Depression Scale,HAD)。结果两组患者术后6h、24h、3d MoCA评分均显著低于术前(P<0.05),术后7d与术前差异不明显(P>0.05),观察组术后6h、24h、3d MoCA评分显著高于对照组(P<0.05);两组术后6h、24h、3d、7d HAD量表焦虑、抑郁评分均显著高于术前,观察组均显著低于对照组(P<0.05)。结论地佐辛用于老年髋关节置换术中辅助麻醉能够降低术后认知功能损害,减轻术后焦虑、抑郁等负性情绪。

Objective To investigate the effect of diazoxide on cognitive function and negative emotions after hip arthroplasty in the elderly.Methods A total of 92 hip arthroplasty patients from February 2020 to March 2023 in our hospital were divided into control group and observation group by coin flip randomization method,45 cases and 47 cases,respectively.In the control group,intraoperative anesthesia was maintained with remifentanil and propofol,and in the observation group,diazoxide was used in combination with the control group.The Montreal Cognitive Assess-ment(MoCA),Hospital Anxiety and Depression Scale(HAD)were compared between the two groups before and 6h,24h,3d and 7d after surgery.Results The 6h,24h,and 3d postoperative MoCA scores were significantly lower than those before surgery in both groups(P<0.05).The difference between 7d postoperatively and preoperatively was not significant(P>0.05).The 6h,24h,and 3d postoperative MoCA scores were significantly higher in the observation group than in the control group(P<0.05).The HAD scale anxiety and depression scores were significantly higher in both groups at 6h,24h,3d,and 7d postoperatively than before surgery,and were significantly lower in the observa-tion group than in the control group(P<0.05).Conclusion Dizocine used as an adjunct to anesthesia in geriatric hip arthroplasty can reduce postoperative cognitive impairment and reduce negative emotions such as postoperative anxiety and depression.
