慢性阻塞性肺疾病病人自我管理能力与家庭功能、应对方式的相关性研究

Correlation of self-management ability with family function and coping style in patients with chronic obstructive pulmonary disease

目的:了解慢性阻塞性肺疾病(COPD)病人自我管理能力,分析家庭功能、应对方式与自我管理能力间的相关性,以期为临床针对COPD病人实施有效的护理干预提供科学的指导。方法:选取115例COPD病人为研究对象,采用一般情况调查表,慢性病自我管理行为量表(CDSMS),家庭关怀度指数量表(APGAR)和医学应对方式量表(MCMQ)对其进行问卷调查,运用Pearson分析法分析COPD病人自我管理能力与家庭功能、应对方式之间的相关关系。结果:年龄≤70岁、有宗教信仰、文化程度>高中、家庭月收入>5000元以及居住情况为与配偶或与父母或子女的COPD病人自我管理能力总分较高,差异均有统计学意义(P<0.05~P<0.01);110例COPD病人自我管理得分(38.06±6.48)分;家庭功能得分(7.12±1.01)分;应对方式得分(47.39±6.23)分;Pearson分析结果显示:自我管理能力与家庭功能、应对方式呈正相关性(P<0.05~P<0.01)。结论:COPD病人自我管理能力、家庭功能与应对方式得分均处于中等水平,且自我管理能力与家庭功能、应对方式均呈正相关。提示临床医护人员应着重关注COPD病人家庭功能与应对方式,针对性地采取措施以增强自我管理能力。

Objective:To understand the self-management ability of patients with chronic obstructive pulmonary disease(COPD),analyze the correlation between family function,coping style and self-management ability,to provide scientific theoretical guidance for clinical implementation of effective nursing intervention for COPD patients.Methods:A total of 115 patients with COPD were selected as study subjects.They were surveyed by the general condition questionnaire,chronic disease self-management behavior scale(CDSMS),family care index scale(APGAR)and medical coping style scale(MCMQ).Pearson analysis was used to analyze the correlation between self-management ability and family function and coping style of COPD patients.Results:COPD patients with age≤70 years old,religious belief,education level>high school,monthly family income>5000 yuan and living with spouse or parents or children had higher scores of self-management ability,and the differences were statistically significant(P<0.05 to P<0.01).The self-management score of 110 COPD patients was(38.06±6.48)points,the family function score was(7.12±1.01)points,and the coping style score was(47.39±6.23)points.Pearson analysis showed that self-management ability was positively correlated with family function and coping style(P<0.05 to P<0.01).Conclusions:The scores of self-management ability,family function and coping style in COPD patients are all at medium level,and self-management ability is positively correlated with family function and coping style.It is suggested that clinical medical staff should pay more attention to family function and coping style of COPD patients and take targeted measures to enhance self-management ability.
