摘要
目的:探究头孢呋辛酯联合小儿豉翘清热颗粒治疗儿童上呼吸道感染的临床效果。方法:选取2020年11月—2022年11月在当阳市人民医院治疗的上呼吸道感染患儿131例,应用随机数字表法将其分为对照组(n=65)及观察组(n=66),对照组接受头孢呋辛酯治疗,观察组接受头孢呋辛酯联合小儿豉翘清热颗粒治疗。对比两组患儿临床症状消失时间、血清免疫球蛋白[免疫球蛋白G(IgG)、免疫球蛋白A(IgA)、免疫球蛋白M(IgM)、免疫球蛋白E(IgE)]、炎症指标[C反应蛋白(CRP)、血清淀粉样蛋白A(SAA)、降钙素原(PCT)]、症状积分(发热、咳嗽、鼻塞)、免疫功能指标(CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))。结果:对照组发热消失时间、咳嗽消失时间、咽痛消失时间、鼻塞消失时间均长于观察组,差异均有统计学意义(P<0.05)。治疗前,两组患儿IgG、IgA、IgM、IgE相较,差异均无统计学意义(P>0.05);治疗后,两组IgE均降低,IgG、IgA、IgM均升高,观察组IgE低于对照组,IgG、IgA、IgM均高于对照组,差异均有统计学意义(P<0.05)。治疗前,两组CRP、SAA、PCT相较,差异均无统计学意义(P>0.05);治疗后,两组CRP、SAA、PCT均降低,观察组均低于对照组,差异均有统计学意义(P<0.05)。治疗前,两组发热积分、咳嗽积分、鼻塞积分相较,差异均无统计学意义(P>0.05);治疗后,两组发热积分、咳嗽积分、鼻塞积分均降低,观察组均低于对照组,差异均有统计学意义(P<0.05)。治疗前,两组CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)相较,差异均无统计学意义(P>0.05);+治疗后,两组CD8均降低,CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)+均升高,观察组CD8低于对照组,CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均高于对照组,差异均有统计学意义(P<0.05)。结论:头孢呋辛酯联合小儿豉翘清热颗粒治疗儿童上呼吸道感染效果显著,可改善免疫球蛋白水平及CRP、SAA、PCT表达水平,缩短症状消失时间,提升机体免疫功能。
Objective:To investigate the clinical efficacy of Cefuroxime Axetil combined with Xiaoer Chiqiao Qingre Granules in the treatment of children with upper respiratory tract infection.Method:A total of 131 children with upper respiratory tract infection treated in Dangyang People's Hospital from November 2020 to November 2022 were selected and divided into control group(n=65)and observation group(n=66)by random number table method.The control group was treated with Cefuroxime Axetil,and the observation group was treated with Cefuroxime Axetil combined with Xiaoer Chiqiao Qingre Granules.The clinical symptom disappearance time,serum immunoglobulin[immunoglobulin G(IgG),immunoglobulin A(IgA),immunoglobulin M(IgM),immunoglobulin E(IgE)],inflammatory indicators[C reactive protein(CRP),serum amyloid A(SAA),procalcitonin(PCT)],symptom score(fever,cough,nasal congestion),immune function indicators(CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+))were compared between the two groups.Result:The disappearance time of fever,cough,sore throat and nasal congestion in control group were longer than those in observation group,the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in IgG,IgA,IgM and IgE between the two groups(P>0.05);after treatment,IgE were decreased,IgG,IgA and IgM were increased in both groups,IgE in the observation group was lower than that in the control group,while IgG,IgA and IgM were higher than those in the control group,the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in CRP,SAA and PCT between the two groups(P>0.05);after treatment,CRP,SAA and PCT were decreased in both groups,and the observation group were lower than those of the control group,the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in fever score,cough score and nasal congestion score between the two groups(P>0.05);after treatment,the scores of fever,cough and nasal congestion in both groups were decreased,and the observation group were lower than those of the control group,the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in CD3^(+),CD4^(+),CD8^(+)and CD4^(+)/CD8^(+)between the two groups(P>0.05);after treatment,CD8^(+)decreased in both groups,while CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)increased in both groups,CD8^(+)in observation group was lower than that in control group,and CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)were higher than those in control group,the differences were statistically significant(P<0.05).Conclusion:Cefuroxime Axetil combined with Xiaoer Chiqiao Qingre Granules has significant effect in the treatment of upper respiratory tract infection in children,which can improve the level of immunoglobulin,the expression levels of CRP,SAA and PCT,shorten the time of symptom disappearance and enhance the immune function of the body.
