摘要
目的探究常规超声结合弹性成像在肉芽肿性小叶性乳腺炎(GLM)与乳腺癌(BC)中鉴别中的应用。方法从2018年1月~2022年1月在本院就诊的GLM和BC患者中各抽取50例作为研究对象,分别命名为GLM组和BC组。所有患者入组后均接受常规超声及弹性成像检查,比较检查结果,评估其在GLM与BC中鉴别中的应用价值。结果BC组腋窝淋巴结增大、BI-RADS 3~6类、AdlerⅠ~Ⅲ级、钙化及后方回声占比均高于GLM组,组间比较有统计学意义(P<0.05);GLM组弥漫性导管扩张、BI-RADS 0~2类、Adler 0级占比高于BC组,组间比较有统计学意义(P<0.05);BC组弹性成像评分高于GLM组,组间比较有统计学意义(P<0.05);常规超声联合弹性成像检测鉴别诊断GLM和BC的约登指数为0.816。结论常规超声结合弹性成像可较为准确地区分GLM与BC,诊断效能佳,应用推广价值高。
Objective To explore the application of conventional ultrasound combined with elastography in the identification of granulomatous lobular mastitis(GLM)and breast cancer(BC).Methods From January 2018 to January 2022,50 GLM and BC patients in our hospital were selected as the research objects,and named as the GLM group and the BC group,respectively.All patients received conventional ultrasound and elastography after enrollment,and the results were compared to evaluate their application value in differentiating GLM from BC.Results The enlarged axillary lymph nodes,BI-RADS category 3~6,Adler gradesⅠ~Ⅲ,calcification and posterior echo in the BC group were higher than those in the GLM group,the comparison between groups was statistically significant(P<0.05).The proportion of diffuse ductal dilatation,BI-RADS category 0~2,and Adler grade 0 in GLM group was higher than that in BC group,the comparison between groups was statistically significant(P<0.05).The elastography score of the BC group was higher than that of the GLM group,the comparison between groups was statistically significant(P<0.05).The Youden index for the differential diagnosis of GLM and BC with conventional ultrasound combined with elastography was 0.816.Conclusion Conventional ultrasound combined with elastography can more accurately distinguish GLM from BC,with good diagnostic efficiency and high application and promotion value.
